摘要
目的 观察CT血管成像(CTA)在髂静脉支架置入术后评估中的应用价值。方法 70例接受髂静脉支架置入术治疗的患者,在其术后随访中分别接受CTA、超声检查检查。以数字减影血管造影(DSA)作为“金标准”,对比两种检查方法对支架上端与髂静脉开口的距离、支架最窄处直径与“金标准”误差,支架位置及形态的诊断符合情况,血栓形成的检出情况。结果 CTA检查对支架上端与髂静脉开口的距离、支架最窄处直径与“金标准”误差分别为(0.67±0.16)、(0.39±0.04)mm,均小于超声检查的(1.53±0.35)、(0.76±0.08)mm,差异具有统计学意义(P<0.05)。CTA检查对支架位置及形态诊断符合率91.43%高于超声检查的67.14%,差异具有统计学意义(P<0.05)。CTA检查对血栓形成的检出率91.67%高于超声检查的58.33%,差异具有统计学意义(P<0.05)。结论 髂静脉支架置入术后随访时进行CTA检查,可明确患者支架位置及形态,明确是否出现血栓形成,及时为评估患者术后恢复情况提供参考依据。
Objective To evaluate the value CT angiography(CTA) in postoperative evaluation of iliac vein stent implantation.Methods 70 patients who underwent iliac vein stent implantation were enrolled.During their postoperative follow-up,CTA and ultrasound examination were performed.Digital subtraction angiography(DSA) was used as the "gold standard" to compare the error of the distance between the upper end of the stent and the opening of the iliac vein,as well as the diameter of the narrowest point of the stent with the "gold standard",diagnostic consistency of stent position and morphology,and detection of thrombosis of the two examination methods were compared.Results The error of CTA for the distance between the upper end of the stent and the opening of the iliac vein,as well as the diameter of the narrowest point of the stent with the "gold standard" were(0.67±0.16) and(0.39±0.04) mm,which were all smaller than(1.53±0.35) and(0.76±0.08) mm of ultrasound examination,and the difference was statistically significant(P<0.05).The consistency rate of CTA examination for stent position and morphology was 91.43%,which was higher than 67.14% of ultrasound examination,and the difference was statistically significant(P<0.05).The detection rate of CTA for thrombosis was 91.67%,which was higher than 58.33% of ultrasound examination,and the difference was statistically significant(P<0.05).Conclusion During the follow-up after iliac vein stent implantation,CTA can clarify the position and morphology of the stent in the patient,determine whether there is thrombosis,and provide a reference basis for the assessment of postoperative recovery in a timely manner.
