摘要
目的:探讨经会阴三维超声测量肛提肌裂孔面积及形态诊断产后子宫脱垂的价值。方法:选取2021年6月至2023年6月收治的100例产后子宫脱垂患者为观察组,以100例产后无脱垂产妇为对照组。均接受经会阴三维超声检查,分析肛提肌裂孔面积及形态诊断子宫脱垂的价值。结果:观察组静息状态下及Valsalva动作下肛提肌裂孔面积、前后径、横径及肛提肌左右支夹角均高于对照组(P<0.05)。Valsalva动作下肛提肌裂孔面积、前后径、横径、左右支夹角诊断子宫脱垂的AUC分别为0.812、0.772、0.965、0.953,高于静息状态下的AUC 0.736、0.651、0.924、0.810(P<0.05)。结论:经会阴三维超声可清晰的观测盆底器官的形态变化情况,通过测量肛提肌裂孔面积及形态变化诊断子宫脱垂的价值较高。
Objective:To evaluate the value of three-dimensional perineal ultrasonography in diagnosing postpartum uterine prolapse by measuring the hiatus area and shape of levator anal muscle.Methods:100 postpartum patients with uterine prolapse admitted to our hospital from June 2021 to June 2023 were selected as the observation group,and 100 postpartum women without prolapse were selected as the control group.All patients underwent transvaginal three-dimensional ultrasonography to analyze the area and shape of the hiatus of the levator anal muscle in the diagnosis of uterine prolapse.Results:The hiatus area,anterior and posterior diameter,transverse diameter and left and right branches of levator anal muscle in observation group were higher than those in control group at rest and under Valsalva movement(P<0.05).The AUC for diagnosis of uterine prolapsis was 0.812,0.772,0.965,0.953,respectively,and higher than the AUC of 0.736,0.651,0.924,0.810 in resting state(P<0.05).Conclusion:Transvaginal three-dimensional ultrasound can clearly observe the morphological changes of pelvic floor organs,and it is valuable to diagnose uterine prolapse by measuring the hiatus area and morphological changes of anal levator muscle.
