中医药调控P2X受体治疗神经病理性疼痛的研究进展

摘要 神经病理性疼痛(NPP)作为一种复杂的慢性疼痛,疼痛程度高、病程长,严重影响患者的生活质量。P2X受体可被三磷酸腺苷激活,并广泛参与神经系统的功能活动,在NPP的发生和发展过程中起关键作用。目前中医药调控P2X受体治疗NPP的研究逐渐深入,并凭借自身多靶点、多通路、防治结合和副作用低等优势引起广泛关注。中医药治疗通过下调P2X受体表达调控多种信号靶点、改善神经损伤、调节神经免疫机制及抗氧化应激,抑制炎症反应、降低神经兴奋性,从而缓解NPP。本文对P2X受体结构及其在NPP发生和发展中的病理机制进行总结,重点阐述中医药调控P2X受体治疗NPP的机制及作用,以期为中医药对NPP的防治及深入研究提供思路和方法。 Neuropathic pain(NPP)is a complex chronic pain with high pain level and long duration,which seriously affects the quality of life of patients.P2X receptor can be activated by adenosine triphosphate,and is widely involved in the functional activities of the nervous system,plays a crucial role in the occurrence and development of NPP.At present,the research of traditional Chinese medicine regulating P2X receptor in the treatment of NPP is gradually deepening,and has attracted wide attention by virtue of its own advantages of multi-targets,multi-pathways,combination of prevention and treatment,and low side effects.Traditional Chinese medicine therapy can modulate multiple signaling targets,improve nerve injury,regulate neuroimmune mechanisms,and counteract oxidative stress through down-regulation of P2X receptor expression,thereby inhibiting inflammatory responses,reducing neural excitability,and alleviating NPP.This article summarizes the structure of P2X receptors and their pathological mechanisms in the occurrence and development of NPP,with a focus on the mechanism and role of traditional Chinese medicine in regulating P2X receptors for the treatment of NPP.The aim is to provide ideas and methods for the prevention,treatment,and in-depth research of NPP by traditional Chinese medicine.
