摘要
目的探讨螺旋断层放疗同步顺铂化疗联合三维后装腔内放疗治疗局部晚期宫颈癌的效果。方法选取2020年10月至2022年10月我院收治的62例局部晚期宫颈癌患者,以治疗方案不同将其分为二维组和三维组,各31例。二维组采用螺旋断层放疗+顺铂化疗+二维后装腔内放疗,三维组采用螺旋断层放疗+顺铂化疗+三维后装腔内放疗。比较两组的治疗效果。结果两组的治疗总有效率无明显差异(P>0.05)。治疗后,三维组的CD4^(+)、CD4^(+)/CD8^(+)及免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)水平高于二维组,CD8^(+)低于二维组(P<0.05)。两组的早期不良反应总发生率无明显差异(P>0.05);三维组的晚期不良反应总发生率显著低于二维组(P<0.05)。结论螺旋断层放疗同步顺铂化疗联合三维后装腔内放疗治疗局部晚期宫颈癌患者的效果较好,能够减轻对患者免疫功能的损伤,降低晚期不良反应发生率。
Objective To investigate the effect of helical tomotherapy combined with cisplatin chemotherapy and three-dimensional afterloading intracavitary radiotherapy in the treatment of locally advanced cervical cancer.Methods A total of 62 patients with locally advanced cervical cancer admitted in our hospital from October 2020 to October 2022 were selected and divided into two-dimensional group and three-dimensional group according to different treatment schemes,with 31 cases in each group.The two-dimensional group was treated with helical tomotherapy+cisplatin chemotherapy+two-dimensional afterloading intracavitary radiotherapy,and the three-dimensional group was treated with helical tomotherapy+cisplatin chemotherapy+three-dimensional afterloading intracavitary radiotherapy.The therapeutic effects of the two groups were compared.Results There was no significant difference in the total effective rate of treatment between the two groups(P>0.05).After treatment,the CD4^(+),CD4^(+)/CD8^(+),immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)levels in the three-dimensional group were higher than those in the two-dimensional group,and CD8^(+)was lower than that in the two-dimensional group(P<0.05).There was no significant difference in the total incidence of early adverse reactions between the two groups(P>0.05);the total incidence of late adverse reactions in the three-dimensional group was significantly lower than that in the two-dimensional group(P<0.05).Conclusion Helical tomotherapy combined with cisplatin chemotherapy and three-dimensional afterloading intracavitary radiotherapy is effective in the treatment of patients with locally advanced cervical cancer,which can reduce the damage to the immune function of patients and decrease the incidence of late adverse reactions.
