摘要
目的构建三级甲等血液专科医院护理工作类别权重指标体系,为护理工作量的计算提供科学可靠的客观数据,推动医院护理垂直管理的开展。方法2023年7月至8月,组建多部门研究组,以“基本医疗服务项目目录”为基础,进行护理工作项目类别的提取。通过文献分析、多家医院现况调查,并结合该院实际情况初步确定护理服务项目类别框架,开展德尔菲专家咨询,并通过层次分析法,将专家函询对护理工作项目的主观评分转化为量化的客观数据,确定护理工作项目指标体系及各指标权重。结果2轮专家函询的有效问卷回收率均为100%,专家权威系数分别为0.905、0.900;肯德尔协调系数为0.100~0.603,除“直接护理时间”维度在两轮一级指标的P>0.05,其他均P<0.01。最终构建包括3个一级指标、24个二级指标以及175个三级指标的三级甲等血液专科医院护理工作类别权重指标体系,且层次分析法构建的矩阵模型具有满意的一致性(CR<0.1)。结论该研究形成的三级甲等血液专科医院护理工作类别权重指标体系科学、可靠,内容全面,具有较强的专科性,可为护理工作量的计算提供客观的数据,为其他医疗机构提供可借鉴的经验。
Objective To construct a nursing work category weight index system in tertiary hematology hospital,so as to provide scientific and reliable objective data for calculating nursing workload and promote the development of hospital nursing vertical management.Methods From July to August 2023,a multi-department research group was formed,using the“directory of essential medical service items”as a foundation to extract nursing work categories.The category framework of nursing service items was preliminarily established based on literature analysis,survey of multiple hospitals,and combined with the actual situation of the hospital under study.Delphi expert consultations were conducted.Through the Analytic Hierarchy Process(AHP),the subjective scores by expert consultaion were converted into quantitative objective data,to determine the index system of nursing work items and the weight of each index.Results The recovery rates of valid questionnaires in the two rounds of expert consultation were both 100%,with expert authority coefficients of 0.905 and 0.900,respectively.The Kendall's W coefficient of concordance ranged from 0.100 to 0.603,except for the dimension of“direct nursing time”in two rounds of level-1 index(P>0.05),the others were P<0.01.The final weighted indicator system comprised three first-level indicators,24 second-level indicators and 175 third-level indicators,with a satisfactory consistency in the matrix model constructed using AHP(CR<0.1).Conclusions The established weighted indicator system for nursing work categories in a tertiary hematology hospital is scientific,reliable,comprehensive and professional.It can provide authoritative data for calculatin nursing workload and provide reference for other medical institutions.
