肺部超声评分联合血清sFlt-1、sIL-2R、sTREM-1对肺部感染儿童病情严重程度的评估价值

摘要 目的分析肺部超声评分联合血清可溶性血管内皮生长因子受体-1(sFlt-1)、可溶性白细胞介素2受体(sIL-2R)、可溶性髓系细胞触发受体-1(sTREM-1)对肺部感染儿童病情严重程度的评估价值。方法选取2020年5月-2023年5月青岛大学附属青岛市海慈医院收治的146例肺部感染患儿为研究对象,根据肺部感染评分(CPIS)分为轻症组88例和重症组58例,另随机选取同期健康儿童150名为健康对照组。比较三组肺部超声评分、血清sFlt-1、sIL-2R、sTREM-1水平,不同病情严重程度肺部感染患儿肺通气功能,绘制受试者工作特征(ROC)曲线分析以上指标单独及联合检测对肺部感染患儿病情严重程度的评估价值。结果健康对照组、轻症组、重症组血清sFlt-1、sIL-2R、sTREM-1水平呈升高趋势(P<0.05)。重症组氧合指数(OI)低于轻症组,肺泡-动脉氧分压差(A-aDO2)、呼吸指数(RI)高于轻症组(P<0.05)。绘制ROC曲线分析肺部超声评分、血清sFlt-1、sIL-2R、sTREM-1水平联合检测对肺部感染患儿病情严重程度的评估价值较高,曲线下面积(AUC)为0.909,敏感度为84.48%,特异度为89.77%。结论肺部超声评分、血清sFlt-1、sIL-2R、sTREM-1随着肺部感染患儿疾病发生、病情严重程度而提高,患儿肺通气功能下降,且联合检测对病情严重程度具有评估价值。 OBJECTIVE To analyze the value of pulmonary ultrasound score combined with serum soluble vascular endothelial growth factor receptor-1(sFlt-1),soluble interleukin-2 receptor(sIL-2R)and soluble triggering receptor expressed on myeloid cells 1(sTREM-1)in assessment of illness condition of the children with pulmonary infection.METHODS A total of 146 children with pulmonary infection who were treated in Qingdao Hiser Hospital Affiliated to Qingdao University from May 2020 to May 2023 were recruited as the research subjects and were divided into the mild group with 88 cases and the severe group with 58 cases according to the clinical pulmonary infection score(CPIS).Meanwhile,150 healthy children were chosen as the healthy control group.The pulmonary ultrasound score,serum sFlt-1,sIL-2R and sTREM-1 levels were compared among the three groups.The pulmonary ventilator function indexes were compared among the children with different severities of pulmonary infection.The values of the single and joint detection of the above indexes in assessment of illness condition were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS The levels of serum sFlt-1,sIL-2R and sTREM-1 of the healthy control group,the mild group and the severe group showed upward trends(P<0.05).The oxygenation index(OI)of the severe group was significantly lower than that of the mild group,the alveola-arterial oxygen partial pressure(A-aDO2)and respiratory index(RI)of the severe group were significantly higher than those of the mild group(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of the joint detection of serum sFlt-1,sIL-2R and sTREM-1 was 0.909 in assessment of illness condition of the children with pulmonary infection,with the sensitivity 84.48%,the specificity 89.77%.CONCLUSIONS The pulmonary ultrasound score,serum sFlt-1,sIL-2R and sTREM-1 levels of the children with pulmonary infection are elevated with the occurrence of disease and illness condition,the pulmonary ventilation function is reduced.The joint
