泰州市大气NO_(2)对儿童门诊量影响的时间序列分析

摘要 目的探讨泰州市大气NO_(2)对儿童门诊就诊量的影响。方法收集泰州市2023年1月1日—12月31日气象资料(包括日平均温度、日平均相对湿度、日平均气压)、环保资料(包括每日PM_(10)、PM_(2.5)、SO_(2)、NO_(2)、CO、O_(3-8 h)平均质量浓度)以及同期某综合性医院儿科门诊就诊数据。通过广义相加模型的时间序列分析方法,分析泰州市大气NO_(2)浓度与同期儿科门诊就诊人次的关系。结果Spearman相关分析显示,大气污染物NO_(2)与其他大气污染物SO_(2)、CO、O_(3-8 h)、PM_(10)和PM_(2.5)均存在正相关(P<0.05)。单污染物模型中,当NO_(2)日均质量浓度每升高10μg/m^(3)时,其对当日呼吸系统疾病门诊量和日门诊总量均产生了明显影响,分别增加0.48%(95%CI:0.18%~0.77%)、0.16%(95%CI:0.04%~0.27%)。多污染物模型中,纳入其他5种污染物(SO_(2)、CO、PM_(10)、PM_(2.5)、O_(3-8 h))后,NO_(2)质量浓度的变化对儿童呼吸系统疾病门诊量和儿童日门诊总量的影响仍有统计学效应(P<0.05)。结论泰州市NO_(2)质量浓度与本次调查的儿童门诊就诊风险增加有相关性,且存在滞后效应。 Objective To understand the effect of atmospheric NO_(2)on visits of the pediatric department in Taizhou City.Methods From January 1,2023 to December 31,2023,the daily meteorological information(daily mean temperature,humidity and atmospheric pressure),and air pollutants(PM_(2.5),PM_(10),SO_(2),NO_(2)and O_(3-8 h))and the daily data of pediatric outpatient visits in a general hospital during the same period.The relationship between NO_(2)concentration and daily pediatric outpatient visits in Taizhou was analyzed by generalized additive model(GAM).Results Spearman correlation analysis showed that air pollutant NO_(2)was positively correlated with other air pollutants SO_(2),CO,O_(3-8 h),PM_(10)and PM_(2.5)(P<0.05).Significant associations were showed between NO_(2)and children daily outpatient volume and respiratory department visiting(lag0),and ERs were 0.16%(95%CI:0.04%-0.27%)and 0.48%(95%CI:0.18%-0.77%)associated with an increase of 10μg/m^(3)NO_(2).After controlling other pollutants(SO_(2),CO,PM_(10),PM_(2.5),O_(3-8 h))simultaneously,significant association were still found between NO_(2)and children daily outpatient amount and respiratory department visits(P<0.05).Conclusion Atmospheric NO_(2)pollution level has a significant impact on visits of the pediatric department in Taizhou City,with a certain lagged effect.
