摘要
目的评价五味消毒饮加减治疗子宫内膜炎急性发作期的临床疗效及对炎症因子和miR-140-5p、TLR4表达的影响。方法将86例患者随机分为对照组与观察组各43例。对照组口服盐酸多西环素片;观察组在对照组治疗的基础上给予五味消毒饮加减内服治疗。记录腹痛消失时间、白带恢复正常时间;进行治疗前后热毒瘀结证评分;检测治疗前后相关炎症介质水平;检测治疗前后miR-140-5p、Toll样受体4(TLR4)水平;进行治疗前后彩色多普勒超声检测,记录血流指数(FI)、阻力指数(RI)和搏动指数(PI);比较临床疗效。结果治疗后,观察组总有效率明显高于对照组(P<0.05);观察组患者主要症状病程短于对照组(P<0.05);观察组主要症状评分和总分明显低于对照组(P<0.05);观察组各炎症介质水平均明显低于对照组(P<0.05);观察组TLR4水平低于对照组,miR-140-5p水平高于对照组(P<0.05);观察组的RI、PI低于对照组,FI高于对照组(P<0.05)。结论五味消毒饮加减治疗子宫内膜炎急性发作期患者,可调节miR-140-5p和TLR4表达,抑制促炎因子释放,减轻炎性损伤,改善子宫内膜血液循环,缩短病程,改善临床症状,提高临床治疗效果,值得临床使用。
Objective:To evaluate the clinical efficacy of Modified Wuwei Xiaodu Decoction in the treatment of acute attack of endometritis and its effect on the expression of inflammatory factors and miR-140-5p,TLR4.Methods:A total of 86 patients with acute phase endometritis were randomly divided into the control group and observation group,43 cases in each group.The control group was given oral doxycycline hydrochloride tablets while the observation group was treated with Modified Wuwei Xiaodu Yin on the basis of the control group.Abdominal pain disappearance time and leucorrhea recovery time were recorded;dampness-heat stasis syndrome score was measured before and after treatment;inflammatory factors,miR-140-5p and Toll-like receptor 4(TLR4)levels were measured before and after treatment;color doppler ultrasonography was performed to record blood flow index(FI),resistance index(RI)and pulsatility index(PI).The clinical efficacy was compared.Results:After treatment,the total effective rate of the observation group was higher than the control group(P<0.05);the course of main symptoms in the observation group was shorter than the control group(P<0.05);the main symptom scores and the total score of dampness-heat stasis syndrome in the observation group was lower than the control group(P<0.05);the inflammatory factors in the observation group were lower than the control group(P<0.05);the TLR4 levels were lower and miR-140-5p levels were higher of the observation group compared with the control group(P<0.05);RI and PI were lower and FI was higher in the observation group compared with the control group(P<0.05).Conclusion:Modified Wuwei Xiaodu Decoction can regulate the expression of miR-140-5p and TLR4,inhibit the release of pro-inflammatory factors,reduce inflammatory injury,improve endometrial blood circulation,shorten the course of disease,improve clinical symptoms,and improve the clinical treatment effect in patients with acute exacerbation of endometritis,which is worthy of clinical use.
