超声微血管成像对老年甲状腺结节诊断价值研究

摘要 目的目的本研究旨在评估超声微血管成像(SMI)技术在老年甲状腺结节良恶性鉴别诊断中的应用价值。方法方法对2023年2月至2023年9月在定西市第一人民医院就诊的100例老年甲状腺结节患者的超声资料进行回顾性研究。通过对照分析常规超声与SMI血流特征的声像图改变,采用多因素logistic回归分析筛选影响老年甲状腺结节恶性的关键因素。结果结果SMI在检测甲状腺结节微血管分布及血流特征方面表现出显著优势。与常规超声相比,SMI能更清晰地显示结节内部的微血管结构和血流模式,对良恶性结节的区分具有更高的敏感性和特异性。恶性结节在SMI下通常呈现丰富且不规则的血流信号,而良性结节则表现为较少且规则的血流信号。多因素logistic回归分析筛选出结节大小、边缘形态、内部回声特征、微钙化点和血流信号强度等作为影响老年甲状腺结节恶性的关键因素。其中,血流信号强度和微钙化点的存在是预测恶性结节的重要指标。数据分析显示,恶性结节的血流信号强度显著高于良性结节,且微钙化点在恶性结节中的出现频率更高。结论结论超声微血管成像技术在老年甲状腺结节的良恶性鉴别诊断中具有重要的临床应用价值,能够显著提高诊断的准确性和可靠性,值得在临床实践中推广应用。 Objective This study aims to evaluate the application value of ultrasound microvascular imaging(SMI)technology in the differ-ential diagnosis of benign and malignant thyroid nodules in elderly patients.Method A retrospective study was conducted on the ultra-sound data of 100 elderly patients with thyroid nodules who visited the First People's Hospital of Dingxi City from February 2023 to Septem-ber 2023.By comparing and analyzing the changes in ultrasound images of conventional ultrasound and SMI blood flow characteristics,mul-tiple logistic regression analysis was used to screen for key factors affecting the malignancy of elderly thyroid nodules.Result SMI showed significant advantages in detecting the microvascular distribution and blood flow characteristics of thyroid nodules.Compared with conven-tional ultrasound,SMI can more clearly display the microvascular structure and blood flow patterns inside nodules,and has higher sensitiv-ity and specificity in distinguishing between benign and malignant nodules.Malignant nodules usually present rich and irregular blood flow signals under SMI,while benign nodules exhibit fewer and regular blood flow signals.Multivariate logistic regression analysis identified nod-ule size,edge morphology,internal echo characteristics,microcalcifications,and blood flow signal intensity as key factors affecting the malignancy of elderly thyroid nodules.Among them,the intensity of blood flow signals and the presence of microcalcifications are important indicators for predicting malignant nodules.Data analysis shows that the blood flow signal intensity of malignant nodules is significantly higher than that of benign nodules,and the frequency of microcalcifications is higher in malignant nodules.Conclusion Ultrasound micro-vascular imaging technology has important clinical application value in the differential diagnosis of benign and malignant thyroid nodules in the elderly,which can significantly improve the accuracy and reliability of diagnosis and is worthy of promotion and application
