摘要
儿童抽动障碍是常见的儿科神经发育障碍性疾病,症状复杂多变,常伴多种情绪和行为问题,严重影响患儿日常学习生活和社会交往,且近年来发病率不断上升。目前西医尚无治疗儿童抽动障碍的特效药物,临床以多巴胺受体拮抗剂等治疗为主,但不良反应较多,家长接受程度较低,严重影响临床应用率及依从性。中医药治疗儿童抽动障碍通过剖析儿童的生理病理特性予以对症治疗,临床疗效显著,得到了广大患者的认可。“龙雷之火”首见于《格致余论》,为肝肾阴阳失衡,相火急剧变动而产生的一种病理之火。肾水不足、水浅不养龙为其病机关键,湿浊弥漫为其发病诱因。济南市中医医院崔文成教授从事中医工作40余年,深入研究“龙雷之火”理论,结合现代小儿体质特点,认为儿童抽动障碍在病性、病位、病理、病势、症状等多方面都与龙雷之火上炎密切相关,故从“龙雷之火”论治儿童抽动障碍。崔教授认为,患病初期以脾虚肝乘,风动痰扰为主,治以祛痰除湿、以消阴翳;后期以阴虚阳亢,龙火升腾为主,治以滋阴潜阳、益水养龙。儿童抽动障碍势如龙雷,故崔教授在辨证论治的基础上常根据患儿的症状体征灵活加减用药。分享案例一则,介绍崔教授基于“龙雷之火”理论治疗儿童抽动障碍经验,以期为临床提供参考。
Children’s tic disorder is a common neurodevelopmental disorder in pediatrics.Its symptoms are complex and changeable,often accompanied by a variety of emotional and behavioral problems,which seriously affects children’s daily learning and social interaction.In recent years,the incidence rate has been rising.At present,there is no specific medication in Western medicine for treating children’s tic disorders.Clinical use mainly focuses on dopamine receptor antagonists,but there are more adverse reactions and lower acceptance by parents,seriously affecting the clinical application rate and compliance.Traditional Chinese medicine treatment for children’s tic disorders has achieved significant clinical efficacy by analyzing the physiological and pathological characteristics of children,and has been recognized by a large number of patients.ministerial fire is first seen in Gezhi Yulun(《格致余论》).It is a pathological fire caused by the imbalance of Yin(阴)and Yang(阳)of liver and kidney,and the rapid change of ministerial fire.The key to its pathogenesis is insufficient kidney water and shallow water that does not nourish the Dragon,while the diffuse dampness and turbidity are the causes of its onset.Professor CUI Wencheng from Ji’nan Hospital of Traditional Chinese Medicine has been engaged in traditional Chinese medicine work for more than 40 years,has conducted in-depth research on the theory of ministerial fire and combined it with the characteristics of modern children’s physical constitution,and believes that children’s tic disorders are closely related to inflammation caused by ministerial fire in various aspects such as disease nature,location,pathology,disease tendency,and symptoms.Therefore,the professor innovatively treats children’s tic disorders from the perspective of ministerial fire.Professor CUI believes that in the early stage,spleen deficiency and liver stagnation are mainly caused by wind and phlegm disturbance.Treatment focuses on removing phlegm and dampness.In the later
