摘要
崔文成教授提出了儿童鼻鼽(过敏性鼻炎)风湿肺热证这一新的病证名称。证候为反复发作的喷嚏、流涕、鼻痒、鼻塞,受风易发,遇冷气突发或加重,或鼻黏膜红肿或鼻衄或鼻下两旁斑烂;或唇红、口干、烦热、大便干燥;或咽干、咽痒、清嗓;或目痒、肤痒,或眼周青紫,或下睑色黯;舌质红,舌苔少,或剥或裂,脉滑或细。病因:禀赋不足,外感风淫邪气,内伤情志与饮食、剧烈运动等。病机:①禀赋薄弱、肾气不足、素体阴虚而水亏金亢,或内伤情志、木火刑金,致使鼻窍内有壅塞之气。②胃强脾弱、脾阳不升、运化失司、气不布津而致湿热郁肺,使鼻窍隔膜有胶固之痰。③鼻窍触受风邪,就是外有非时之感。以上三者相合,闭拒气道之鼻窍,发为鼻鼽。治法:一要疏外风而通鼻窍,二要清肺热而息肝风,三要升阳除湿而祛胶固之痰,四要护阴滋润及鼻窍黏膜,五要适时增减衣物、保持体温而使气化复常。辛夷清肺藿胆方奏祛风通窍,滋润清肺,除湿祛痰之功效,使肺气宣发肃降及输布复常,从而使风邪得疏、壅气得通、痰湿得祛、肺热得清、窍道得润,诸症得止。文章介绍了崔文成用辛夷清肺藿胆方治疗儿童鼻鼽风湿肺热证经验,为临床提供参考。
Professor CUI Wencheng proposed the new name of the wind-damp lung heat syndrome,which is called allergic rhinitis in children.Syndrome:repeated sneezing,runny nose,nasal itching,nasal congestion,easy to catch wind,sudden attack or aggravated in case of cold air.The nasal mucosa is red and swollen,or epistaxis,or spots on both sides of the nose.The patients have red lips,dry mouth,irritated heat and dry stool.There hare dry throat,itchy throat,clear throat.There hare itchy eyes,itchy skin,or bluish purple around the eyes,or dark lower eyelid.The tongue is red with little coating,or peeled or cracked,and the veins are smooth or fine.Etiology:insufficient endowment,exogenous wind and evil,internal injury of emotion and diet,strenuous exercise,etc..Pathogenesis:①Weak endowment,insufficient kidney Qi(气),Yin(阴)deficiency of the body,water deficiency and hyperactivity of gold,or internal injury of emotion,wood fire and gold,resulting in obstruction of Qi in the nostrils.②The stomach is strong and the spleen is weak,the spleen Yang(阳)is not raised,the transportation is lost,the Qi is not distributed and the fluid is not distributed,and the lung is damp and hot,which causes the nasal orifice diaphragm sticky phlegm.③Once the nostrils are exposed to wind evil,there is a sense of impermanence.When these three things are in harmony,the nose orifices of the airways will be closed,causing allergic rhinitis.Treatment:First,it is necessary to dredge the external wind and open the nostrils.Second,it is necessary to clear the lung heat and extinguish the liver wind.Third,it is necessary to raise the Yang and remove the dampness and phlegm.Fourth,it is necessary to nourish the Yin and nasal cavity mucosa.Fifth,it is necessary to increase and reduce clothing and maintain the body temperature at the right time to make the Qi return to normal.The Xinyi Qingfei Huodan prescription(辛夷清肺藿胆方)can dispel wind and dredge the orifices,moisturize the lungs,and remove dampness and phlegm,so that the lung Qi can be rel
