摘要
目的评价固本保真汤治疗气阴两虚型癌因性疲乏(cancer-related fatigue,CRF)临床疗效。方法将60例气阴两虚型CRF患者随机分为治疗组和对照组。对照组予基础治疗,指导患者合理作息、适当运动锻炼、心理指导等,必要时予以止痛、止呕、纠正水电解质紊乱等对症处理。治疗组在对照组的基础上给予固本保真汤治疗。比较两组治疗前后Piper疲乏量表、卡氏评分量表、中医证候积分和免疫指标变化情况,对疗效数据进行分析总结。结果①Piper疲乏量表比较:治疗组和对照组在第4周和第8周均出现显著改善(P<0.05),但治疗组优于对照组(P<0.05)。②卡氏评分量表比较:治疗组第4周评分变化无统计学意义,第8周时评分变化有统计学意义(P<0.05);对照组在第4周和第8周评分变化均无统计学意义(P>0.05)。③中医证候积分比较:治疗8周后治疗组的总有效率为86.67%(26/30),对照组的总有效率为26.67%(8/30),治疗组显著优于对照组,差异有统计学意义(P<0.05)。④免疫指标比较:治疗组CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)、自然杀伤细胞(natural killer cell,NK)均有所升高,其中CD_(4)^(+)、CD_(8)^(+)的升高具有统计学意义(P<0.05)。对照组的CD_(8)^(+)较前升高具有统计学意义(P<0.05),余均无改善。提示治疗组的免疫指标改善优于对照组。结论固本保真汤在治疗气阴两虚型癌因性疲乏疗效确切,患者疲乏、体力状况均有良好改善作用,且安全性良好,值得临床推广和使用。
Objective To evaluate the clinical efficacy of Guben Baozhen Decoction(固本保真汤)treating cancer-related fatigue(CRF)of Qi-Yin deficiency type.Methods Sixty patients with Qi-Yin deficiency type of CRF were randomly divided into a treatment group and a control group.The control group received basic treatment,including guidance on reasonable rest,appropriate exercise,psychological counseling and symptomatic treatment such as pain relief,antiemetic and correction of electrolyte imbalance if necessary.The treatment group received Guben Baozhen Decoction in addition to the basic treatment.Piper Fatigue Scale(PFS),Karnofsky Performance Status(KPS),TCM syndrome scores and immune indexes were compared between the two groups before and after treatment.Results①Results of PFS comparison:The treatment group and the control group both showed significant improvement at the 4th and 8th week(P<0.05),but the former was better than the latter(P<0.05).②Results of KPS comparison:In the treatment group,the score changes were statistically significant at the 8th week(P<0.05),while in the control group at the 4th and 8th week,there was no significant difference(P>0.05).③Results of TCM syndrome score comparison:After 8 weeks of treatment,the total effective rate of the treatment group was 86.67%(26/30),and that of the control group was 26.67%(8/30).The treatment group was significantly better than the control group,with a statistically significant difference(P<0.05).④Results of immune index comparison:The levels of CD_(3)^(+),CD_(4)^(+),CD_(8)^(+)and natural killer(NK)cells in the treatment group were increased,and the increase of CD_(4)^(+)and CD_(8)^(+)was statistically significant(P<0.05).The CD_(8)^(+)level in the control group was significantly higher than those before(P<0.05),but the rest were not.The results showed that the immune indexes of the treatment group were improved compared to those of the control group.Conclusion Guben Baozhen Decoction has a definite therapeutic effect on Qi-Yin deficiency type of CRF,an
