摘要
目的分析血管超声检查对锁骨下动脉狭窄(subclavian artery stenosis,SAS)合并椎动脉狭窄(vertebral artery stenosis,VAS)的诊断价值。方法选择梁平区中医医院于2021年1月—2023年8月收治的158例疑似SAS合并VAS患者为研究对象,所有患者入院后均接受数字减影血管成像(digital subtraction angiography,DSA)和血管超声检查。以DSA为诊断金标准。评估血管超声诊断SAS合并VAS的诊断效能值(灵敏度、特异度、准确度)、动脉狭窄程度(轻度、中度、重度、闭塞)及盗血情况(Ⅰ型、Ⅱ型、Ⅲ型)的诊断符合率。结果DSA诊断132例SAS合并VAS;血管超声诊断133例SAS合并VAS。血管超声的诊断灵敏度为96.21%(127/132),特异度为76.92%(20/26),准确度为93.04%(147/158),kappa值为0.646。DSA诊断SAS合并VAS轻度狭窄21例,中度狭窄69例,重度狭窄36例,闭塞6例;血管超声分别诊断20例、67例、35例、5例。DSA诊断盗血106例;血管超声诊断盗血103例。血管超声对Ⅰ、Ⅱ、Ⅲ型盗血的诊断符合率分别为100.00%(51/51)和97.87%(46/47)、75.00%(6/8)。结论血管超声检查可用于诊断SAS合并VAS,且诊断价值较高。
Objective To analyze the value of ultrasonography in the diagnosis of subclavian artery stenosis(SAS)combined with vertebral artery stenosis(VAS).Methods A total of 158 patients with suspected SAS combined with VAS admitted to Liangping District Hospital of Traditional Chinese Medicine from January 2021 to August 2023 were selected as the study objects.All patients received digital subtraction angiography(DSA)and vascular ultrasonography after admission.DSA is the gold standard for diagnosis.The diagnostic efficacy(sensitivity,specificity,accuracy),the degree of artery stenosis(mild,moderate,severe,occlusion)and the diagnostic coincidence rate of blood steal(typeⅠ,typeⅡ,typeⅢ)of vascular ultrasound in the diagnosis of SAS combined with VAS were evaluated.Results DSA diagnosed 132 cases of SAS combined with VAS,133 cases of vascular ultrasound diagnosed SAS combined with VAS.The diagnostic sensitivity of vascular ultrasound was 96.21%(127/132),the specificity was 76.92%(20/26),and the accuracy was 93.04%(147/158),the kappa value was 0.646.DSA diagnosis of SAS combined with VAS mild stenosis in 21 cases,moderate stenosis in 69 cases,severe stenosis in 36 cases,occlusion in 6 cases,vascular ultrasound diagnosis in 20 cases,67 cases,35 cases,5 cases.DSA diagnosed 106 cases of blood steed,and vascular ultrasound diagnosed 103 cases of blood steed.The diagnostic coincidence rate of vascular ultrasound forⅠ,Ⅱ,Ⅲtypes of stolen blood was 100.00%(51/51),97.87%(46/47)and 75.00%(6/8)respectively.Conclusion Vascular ultrasonography can be used in the diagnosis of SAS combined with VAS,and the diagnostic value is high.
