摘要
目的 探究临床中针对甲状腺微小乳头状癌实施术中冰冻切片病理检查的诊断价值。方法 方便选取2023年5月—2024年5月漳浦县医院收治的88例疑似甲状腺微小乳头状癌患者为研究对象。以石蜡切片病理作为金标准,同时进行术中冰冻切片病理检查,分析术中冰冻切片病理检查诊断甲状腺微小乳头状癌的诊断效能,并分析甲状腺微小乳头状癌的病理学类型。结果 术后石蜡切片病理诊断结果阳性65例,阴性23例,术中冰冻切片病理诊断真阳性64例,真阴性22例。术中冰冻切片病理诊断和术后石蜡切片病理诊断的kappa值为0.941。术中冰冻切片病理诊断甲状腺微小乳头状癌的准确度为97.73%(86/88)、敏感度为98.46%(64/65)、特异度为95.65%(22/23)、阳性预测值为98.46%(64/65)、阴性预测值为95.65%(22/23)。65例阳性患者中,病理学分型表现为经典型者60例、滤泡亚型者3例、弥漫硬化型者2例。结论 临床中针对甲状腺微小乳头状癌进行术中冰冻切片病理诊断的准确率较高,与术后石蜡切片病理诊断的一致性较高,临床中需根据患者的疾病特点和手术需求选用合理的诊断方法。
Objective To explore the diagnostic value of intraoperative frozen section pathological examination for thy⁃roid micropapillary carcinoma in clinical practice.Methods Eighty-eight cases of suspected thyroid micropapillary carcinoma admitted to Zhangpu County Hospital from May 2023 to May 2024 were conveniently selected as the re⁃search objects.Taking paraffin section pathology as the gold standard,intraoperative frozen section pathological exami⁃nation was simultaneously performed.The diagnostic efficacy of intraoperative frozen section pathological examination for diagnosing thyroid micropapillary carcinoma was analyzed,and the pathological types of thyroid micropapillary car⁃cinoma were also analyzed.Results The postoperative paraffin section pathological diagnosis results were positive in 65 cases and negative in 23 cases.The intraoperative frozen section pathological diagnosis showed true positive in 64 cases and true negative in 22 cases.The kappa value of intraoperative frozen section pathological diagnosis and post⁃operative paraffin section pathological diagnosis was 0.941.The accuracy of intraoperative frozen section pathological diagnosis of thyroid micropapillary carcinoma was 97.73%(86/88),the sensitivity was 98.46%(64/65),the specificity was 95.65%(22/23),the positive predictive value was 98.46%(64/65),and the negative predictive value was 95.65%(22/23).Among the 65 positive patients,the pathological classification showed 60 cases of classic type,3 cases of fol⁃licular subtype,and 2 cases of diffuse sclerosing type.Conclusion Intraoperative frozen section pathological diagnosis of thyroid micropapillary carcinoma in clinical practice has a relatively high accuracy and high consistency with post⁃operative paraffin section pathological diagnosis.In clinical practice,a reasonable diagnostic method should be se⁃lected according to the disease characteristics and surgical needs of patients.
