i-PRF支架和血凝块支架在年轻恒牙感染根管牙髓血管再生治疗中的临床应用及对牙髓电活力的影响

摘要 目的分析注射型富血小板纤维蛋白(i-PRF)支架和血凝块支架在年轻恒牙感染根管牙髓血管再生治疗中的临床应用。方法选取2018年1月至2021年12月收治的60例年轻恒牙牙髓感染患者为研究对象,依据入院编号的奇偶性将其分为对照组和观察组,每组30例。所有患者均行髓腔封药抗感染治疗后牙髓血管再生术治疗,对照组使用血凝块支架,观察组使用i-PRF支架。比较两组的治疗效果。结果观察组的治疗总有效率及牙髓电活力测试阳性率高于对照组,但差异无统计学意义(P>0.05)。术后6个月,观察组的根管壁厚度、牙根长度、恒牙萌出高度大于对照组,根尖孔直径小于对照组(P<0.05)。术后6个月,观察组的牙周咬合力、牙齿咀嚼效率高于对照组,牙龈指数(GI)、牙龈出血指数(BI)、菌斑指数(PLI)评分低于对照组(P<0.05)。结论在年轻恒牙感染根管牙髓血管再生治疗中,i-PRF支架可显著促进恒牙的生长,提升患者的口腔功能,建议临床应用推广。 Objective To analyze the clinical application of injectable platelet-rich fibrin(i-PRF)scaffolds and blood clot scaffolds in pulp revascularization for immature teeth with apical periodontitis.Methods A total of 60 patients with pulp infection of immature teeth admitted from January 2018 to December 2021 were selected as the research objects.According to the parity of admission number,the patients were divided into control group and observation group,with 30 cases in each group.All patients were treated with pulp revascularization after anti-infection treatment with pulp cavity sealing.The control group used blood clot scaffolds,and the observation group used i-PRF scaffolds.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment and the positive rate of pulp electrical activity test in the observation group were higher than those in the control group,but the differences were not statistically significant(P>0.05).At 6 months after operation,the thickness of root canal wall,root length and eruption height of permanent teeth in the observation group were greater than those in the control group,and the diameter of apical foramen was smaller than that in the control group(P<0.05).At 6 months after operation,the periodontal bite force and tooth chewing efficiency of the observation group were higher than those of the control group,and the gingival index(GI),bleeding index(BI)and plaque index(PLI)scores were lower than those of the control group(P<0.05).Conclusion In the treatment of pulp revascularization for immature teeth with apical periodontitis,i-PRF scaffolds can significantly promote the growth of permanent teeth and improve the oral function of patients,which is recommended for clinical application and promotion.
