摘要
目的探讨急性脑梗死患者的衰弱状态与血清指标的相关性。方法选取2021年1月至2023年12月急性脑梗死患者146例,采用Edmonton量表将患者分为为衰弱组70例及无衰弱组76例。分析两组患者血清指标差异;同时采用临床衰弱量表(CFS)将衰弱患者分为轻度、中度、重度衰弱组,比较3组患者血清指标水平差异。结果衰弱组较无衰弱组血红蛋白、甘油三酯、低密度脂蛋白、白蛋白水平均降低(P<0.05),C反应蛋白(CRP)、D-二聚体、同型半胱氨酸水平均升高(P<0.05)。Logistic回归分析发现血红蛋白、同型半胱氨酸指标是患者衰弱的独立危险因素,ROC曲线下面积分别为0.707、0.751。重度衰弱组患者血红蛋白水平低于轻中度衰弱组患者,而CRP、同型半胱氨酸水平增高(P<0.05)。结论血清指标血红蛋白和同型半胱氨酸水平对急性脑梗死衰弱患者的诊断有一定价值。
Objective To explore the frailty status of patients with acute cerebral infarction and assess the diagnostic value of their serum marker levels for frailty.Methods 1146 patients with acute cerebral infarction who visited our hospital from January 2021 to December 2023 were selected.The Edmonton scale was used to assess 70 patients in the frailty group and 76 patients in the non frailty group.The differences in serological indicators between the two groups of patients were analyzed.At the same time,the Clinical Frailty Scale(CFS)was used to divide frailty patients into mild,moderate,and severe frailty groups,and the differences in serum marker levels among the three groups of patients were compared.Results The levels of hemoglobin,triglycerides,low-density lipoprotein,and albumin in the weakened group were lower than those in the non weakened group(P<0.05),while the levels of C-reactive protein(CRP),D-dimer,and homocysteine were higher(P<0.05).Logistic regression analysis found that hemoglobin and homocysteine indicators are independent risk factors for patient frailty,with receiver operating characteristic(ROC)curve areas of 0.707 and 0.751,respectively.The hemoglobin level in patients with severe frailty was lower than that in patients with mild to moderate frailty,while the levels of CRP and homocysteine were increased(P<0.05).Conclusion The levels of serum markers hemoglobin and homocysteine have certain value in the diagnosis of acute cerebral infarction frailty in patients.
