摘要
目的探讨螺旋CT联合血清基质金属蛋白酶(MMP-2)、骨膜蛋白(Periostin)在肺癌早期诊断中的临床价值。方法选取于本院治疗的81例肺癌早期患者作为肺癌组,76例肺部良性病变患者为良性病变组,80例健康体检者为健康组(2020年12月~2023年11月)。比较肺癌组、良性病变组和健康组血清MMP-2、Periostin水平,ROC曲线分析血清MMP-2、Periostin在肺癌早期诊断中的价值,四格表法分析螺旋CT联合血清MMP-2、Periostin对肺癌早期的诊断价值。结果肺癌组血清MMP-2、Periostin水平高于良性病变组和健康组(P<0.05);血清MMP-2对肺癌早期诊断的曲线下面积(AUC)为0.754,最佳截断值为39.11 pg/mL,敏感度为75.31%,特异度为68.42%;血清Periostin对肺癌早期诊断的AUC为0.792,最佳截断值为40.63 ng/L,敏感度为77.78%,特异度为69.74%;螺旋CT检查对肺癌早期诊断的准确率为76.43%,敏感度为76.54%,特异度为76.32%;螺旋CT联合血清MMP-2、Periostin对肺癌早期诊断的准确率为85.35%,敏感度为93.83%,特异度为76.32%,三项联合的准确率、敏感度高于各指标单独诊断(P<0.05)。结论肺癌早期患者血清MMP-2、Periostin水平升高,螺旋CT联合血清MMP-2、Periostin对肺癌早期诊断准确率、敏感性优于各指标单独诊断。
Objective To explore the clinical value of spiral CT combined with serum matrix metalloproteinase(MMP-2)and Periostin in the early diagnosis of lung cancer.Methods A total of 81 early-stage lung cancer patients treated in our hospital were regarded as the lung cancer group,76 patients with benign lung lesions were collected as the benign lung lesion group,and 80 healthy individuals were included as the healthy group(December 2020 to November 2023).The serum MMP-2 and Periostin levels were compared between the lung cancer group,benign lesion group,and healthy group,The ROC curve was applied to analyze the value of serum MMP-2 and Periostin in the early diagnosis of lung cancer,four grid table method was applied to analyze the diagnostic value of spiral CT combined with serum MMP-2 and Periostin for the early stage of lung cancer.Results The serum levels of MMP-2 and Periostin in the lung cancer group were higher than those in the benign lesion group and the healthy group(P<0.05);the area under the curve(AUC)of serum MMP-2 for early diagnosis of lung cancer was 0.754,with an optimal cut-off value of 39.11 pg/mL,sensitivity of 75.31%,and specificity of 68.42%;the AUC of serum Periostin for early diagnosis of lung cancer was 0.792,with an optimal cut-off value of 40.63 ng/L,the sensitivity of 77.78%,and specificity of 69.74%;the accuracy,sensitivity,and specificity of spiral CT in the early diagnosis of lung cancer were 76.43%,76.54%,and 76.32%,respectively;the accuracy,sensitivity,and specificity of early diagnosis of lung cancer using spiral CT combined with serum MMP-2 and Periostin were 85.35%,93.83%,and 76.32%,respectively,the combined accuracy and sensitivity of the three indicators were higher than those of individual diagnosis(P<0.05).Conclusion The serum levels of MMP-2 and Periostin are elevated in early lung cancer patients,and the combination of spiral CT and serum MMP-2 and Periostin has better accuracy and sensitivity in the early diagnosis of lung cancer than individual diagnoses.
