血清Caspase-8、sICAM-1表达与动脉瘤性蛛网膜下腔出血患者介入栓塞术后脑血管痉挛的相关性分析及预测价值

摘要 目的探讨血清半胱氨酸天冬氨酸蛋白水解酶-8(Caspase-8)、可溶性细胞间黏附分子-1(sICAM-1)表达与动脉瘤性蛛网膜下腔出血患者介入栓塞术后脑血管痉挛的关系。方法选取2019年8月至2021年10月间新乡医学院附属南阳市第二人民医院收治的动脉瘤性蛛网膜下腔出血患者208例,根据介入栓塞术后不同程度脑血管痉挛将患者分为无脑血管痉挛组121例,轻度脑血管痉挛组25例,中度脑血管痉挛组42例,重度脑血管痉挛组20例。酶联免疫吸附法测定各组患者介入栓塞术前、术后3 d、术后7 d的血清Caspase-8、sICAM-1水平。结果动脉瘤性蛛网膜下腔出血患者介入栓塞术后3 d、术后7 d血清Caspase-8、sICAM-1水平均明显高于术前,术后7 d血清Caspase-8、sICAM-1水平明显低于术后3 d,差异有统计学意义(P<0.05)。组间比较,术后3 d和术后7 d,重度脑血管痉挛组患者血清Caspase-8、sICAM-1水平明显高于中度脑血管痉挛组和轻度脑血管痉挛组,轻度脑血管痉挛组、中度脑血管痉挛组以及重度脑血管痉挛组患者血清Caspase-8、sICAM-1水平均明显高于无脑血管痉挛组,差异有统计学意义(P<0.05)。Pearson相关性分析显示,介入栓塞术后3 d、7 d的血清Caspase-8与sICAM-1呈正相关。ROC曲线分析显示,介入栓塞术后3 d血清Caspase-8、sICAM-1单独和联合检测在预测脑血管痉挛方面均具有较高的应用价值(P<0.01);其中Caspase-8与sICAM-1联合检测的AUC值和灵敏度最高。结论介入栓塞术后脑血管痉挛的动脉瘤性蛛网膜下腔出血患者血清Caspase-8、sICAM-1表达水平明显增加,联合检测对脑血管痉挛有较高的预测价值。 Objective To investigate the relationship between the expression of serum Caspase-8and soluble intercellular adhesion molecule-1(sICAM-1)and cerebral vasospasm after interventional embolization in patients with aneurysmal subarachnoid hemorrhage.Methods 208patients with aneurysmal subarachnoid hemorrhage treated in our hospital from August 2019to October 2021were selected.According to different degrees of cerebral vasospasm after interventional embolization,the patients were divided into 121cases in the non cerebral vasospasm group,25cases in the mild cerebral vasospasm group,42cases in the moderate cerebral vasospasm group and 20cases in the severe cerebral vasospasm group.5mL of fasting elbow venous blood was collected before,3days and 7days after interventional embolization.The levels of serum Caspase-8and sICAM-1were measured by enzyme-linked immunosorbent assay.Results The levels of serum Caspase-8and sICAM-1were significantly higher at 3days and 7days after operation than those before operation,and the levels of serum Caspase-8and sICAM-1at 7days after operation were significantly lower than those at 3days after operation(P<0.05).Compared among groups,the serum Caspase-8and sICAM-1levels of patients with severe cerebral vasospasm were significantly higher than those of patients with moderate cerebral vasospasm and mild cerebral vasospasm at 3days and 7days after operation,and the serum Caspase-8and sICAM-1levels of patients with mild cerebral vasospasm,moderate cerebral vasospasm and severe cerebral vasospasm were significantly higher than those of patients without cerebral vasospasm,with statistical significance(P<0.05).Pearson correlation analysis showed that serum Caspase-8was positively correlated with sICAM-13and 7days after interventional embolization.ROC curve analysis showed that serum Caspase-8and sICAM-1were of high application value in predicting cerebral vasospasm(P<0.01)3days after interventional embolization;The AUC value and sensitivity of Caspase-8and sICAM-1were the highest.Conclusion The e
