孟德尔随机化研究外周血脂水平与体质量指数对白内障发病风险的影响

摘要 目的探讨体质量指数(BMI)与外周血脂水平对白内障发病风险的影响。方法从人类全基因组关联研究(GWAS)中提取BMI、7种外周血脂指标及4种次要指标工具变量。运用孟德尔随机化(MR)的逆方差加权法(IVW)、MR-PRESSO和MR-Egger方法,探讨上述暴露对老年性白内障(ARC)和其他白内障发病风险的影响,采用MR-PRESSO、Cochran′s Q和MR-Egger intercept检验进行敏感度分析。结果BMI水平升高与ARC风险增加相关,IVW法MR-PRESSO法OR值(95%CI)分别为1.142(1.068~1.220)、1.137(1.068~1.211)(P值均<0.001),与其他白内障风险增加间具有名义上相关性,OR值(95%CI)均为1.115(1.014~1.226)(P值分别为0.025、0.026);未发现血脂指标与白内障风险存在相关性(P值均>0.05)。次要指标中外周C反应蛋白(CRP)水平升高与ARC风险增加间具有名义上的相关性,OR值(95%CI)分别为1.057(1.011~1.106)、1.057(1.011~1.106)、1.067(1.008~1.130)(P值分别为0.014、0.015、0.028)。未发现白蛋白、总胆红素和血尿酸与白内障发病风险间的相关性(P值均>0.05)。逐一调整TG、LDL-C、ApoB和Lp(a)后,BMI水平升高仍与ARC风险升高相关,OR值(95%CI)分别为1.177(1.090~1.272)、1.164(1.083~1.251)、1.245(1.015~1.529)(P值均<0.05)。敏感度分析未发现显著的水平多效性。结论BMI升高和炎症可能参与了白内障的发生与发展,外周血脂代谢可能一定程度上参与了致病过程。 Objective To examine the impact of body mass index(BMI)and peripheral blood lipid levels on risk of cataract.Methods BMI,7 peripheral blood lipid parameters and 4 secondary indicator instrumental variables were extracted from genome-wide association studies(GWAS).In addition,the effects of BMI,peripheral blood lipid parameters and secondary indicators on the risk of age-related cataract(ARC)and other types of cataract were examined using inverse-variance weighted(IVW),Mendelian randomization pleiotropy residual sum and outlier(MR-PRESSO)and Mendelian randomization-Egger(MR-Egger)methods based on Mendelian randomization,and sensitivity analysis was performed with MR-PRESSO,Cochran′s Q test and ME-Egger intercept test.Results ElevatedBMI significantly correlated with an increased risk of ARC,with ORs(95%CI)of 1.142(1.068,1.220)and 1.137(1.068,1.211)for the IVW and MR-PRESSO methods,and nominally correlated with an increased risk of other types of cataract,with an OR of 1.115(1.014,1.226)for both methods(P=0.025 and 0.026).However,no associations were detected between blood lipid levels and the risk of cataract(all P values>0.05).Of secondary indicators,elevated C-reactive protein(CRP)levels nominally correlated with an increased risk ofARC,with ORs(95%CI)of 1.057(1.011,1.106),1.057(1.011,1.106)and 1.067(1.008,1.130)(P=0.014,0.015 and 0.028),and no associations of albumin,total bilirubin or blood uric acid were detected with the risk of cataract(all P values>0.05).After adjustment for triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),apolipoprotein B(ApoB)and lipoprotein(a)(Lp[a])step by step,elevated BMI still correlated an increased risk of ARC,with ORs(95%CI)of 1.177(1.090,1.272),1.164(1.083,1.251)and 1.245(1.015,1.529)(all P values<0.05),respectively.Sensitivity analysis detected no significant horizontal pleiotropy.Conclusions Elevated BMI and inflammation may be involved in the development and progression of cataract,and peripheral blood lipid metabolism may contribute to the pathogenesis of catar
