摘要
目的探讨温肾强督方对强直性脊柱炎患者骨代谢及炎症细胞因子的影响。方法将医院强直性脊柱炎患者71例随机分组,对照组34例采用柳氮磺吡啶、双氯芬酸钠治疗;在此基础上,观察组37例采用温肾强督方治疗,每日1剂,持续治疗8周。比较两组临床疗效,记录血肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、血沉(erythrocyte sedimentation rate,ESR)、C反应蛋白(C-reactive protein,CRP)、骨钙素(osteocalcin,BGP)、I型胶原C末端肽(C-terminal peptide of type I collagen,CTX)、骨碱性磷酸酶(Bone alkaline phosphatase,BALP)及腰椎L 1-4、股骨颈的骨密度。结果与对照组比,观察组治疗后血TNF-α、ESR及CRP水平较低,血BGP、BALP水平较高,血清CTX水平较低,腰椎、股骨颈骨密度较高,差异有统计学意义(P<0.05)。对照组有效率76.47%低于观察组94.59%,差异有统计学意义(P<0.05)。两组不良反应比较,差异无统计学意义(P>0.05)。结论温肾强督方对强直性脊柱炎患者的疗效显著,通过改善骨代谢和炎症细胞因子,以避免骨质流失,值得推广。
Objective To explore the effects of Wenshen Qiangdu Formula(温肾强督方)on bone metabolism and inflammatory cytokines in patients with ankylosing spondylitis.Method:Seventy-one patients with ankylosing spondylitis in the hospital were randomly divided into two groups.The control group consisted of 34 patients treated with sulfasalazine and diclofenac sodium.On this basis,37 cases in the observation group were treated with Wenshen Qiangdu Formula,taking one dose a day for 8 weeks.The clinical efficacy of two groups was compared and the blood tumor necrosis factor-α(TNF-α),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),osteocalcin(BGP),C-terminal peptide of type I collagen(CTX),bone alkaline phosphatase(BALP)and bone mineral density of lumbar L1-4 and neck of femur recorded.Result:Compared with those of the control group,the levels of TNF-α,ESR and CRP of the observation group were lower,the levels ofBGP and BALP in blood were higher,serum CTX levels were lower and bone mineral density of lumbar vertebrae and neck of femur were higher,and the difference was statistically significant(P<0.05).The effective rate(76.47%)in the control group was lower than that(94.59%)in the observation group,with a statistically significant difference(P<0.05).There was no statistically significant difference in adverse reactions between the two groups(P>0.05).Conclusion Wenshen Qiangdu Formula has a significant therapeutic effect on patients with ankylosing spondylitis.By improving bone metabolism and inflammatory cytokines,it can avoid bone loss and is worthy of promoting.
