摘要
目的探讨心肌梗死患者血清微小RNA(miR)-34a-5p、C1q/肿瘤坏死因子相关蛋白-9(CTRP9)水平及其与心肌重构的相关性。方法选取2019年3月至2021年3月于该院治疗的急性心肌梗死患者100例作为观察组,另选取同期在该院体检的健康体检者100例作为对照组;采用实时荧光定量聚合酶链反应(qRT-PCR)检测血清中miR-34a-5p水平;采用酶联免疫吸附试验检测血清中CTRP9水平;通过心脏彩色超声诊断仪检测心肌重构指标[左心房内径(LAD)、左心室重构指数(LVRI)、左心室质量指数(LVMI)、左心室后壁厚度(LVPW)];采用Pearson相关分析血清miR-34a-5p与CTRP9的相关性及二者与心肌重构指标的相关性;采用多因素Logistic回归分析急性心肌梗死发生的影响因素。结果与对照组相比,观察组血清miR-34a-5p水平、LVPW、LVMI、LAD均明显增加(P<0.05),CTRP9水平、LVRI均明显降低(P<0.05)。Pearson相关分析结果显示,miR-34a-5p与CTRP9、LVRI呈负相关(r=-0.572、-0.645,P<0.05),与LVPW、LVMI、LAD呈正相关(r=0.418、0.420、0.569,P<0.05);CTRP9与LVRI呈正相关(r=0.424,P<0.05),与LVPW、LVMI、LAD均呈负相关(r=-0.583、-0.469、-0.518,P<0.05)。多因素Logistic回归分析结果显示,miR-34a-5p水平、LVPW、LVMI、LAD、LVRI、CTRP9水平均是急性心肌梗死发生的影响因素(P<0.05)。结论在急性心肌梗死患者血清中miR-34a-5p水平明显增加,CTRP9水平明显降低,二者呈负相关,且与心肌重构密切相关,是急性心肌梗死发生的影响因素。
Objective To investigate the levels of serum microRNA(miR)-34a-5p and C1q/tumor necrosis factor associated protein-9(CTRP9)in the patients with myocardial infarction and their correlation with myocardial remodeling.Methods A total of 100 patients with acute myocardial infarction treated in this hospital from March 2019 to March 2021 were selected as the observation group.Other 100 healthy examinees who underwent physical examinations in this hospital during the same period were selected as the control group.The level of serum miR-34a-5p was detected by real time fluorescence quantitative polymerase chain reaction(qRT-PCR)method;the level of serum CTRP9 was detected by enzyme-linked immunosorbent assay;the cardiac color ultrasound diagnostic instrument was used to detect the myocardial remodeling indicators[left atrial inner diameter(LAD),left ventricular remodeling index(LVRI),left ventricular mass index(LVMI),left ventricular posterior wall thickness(LVPW)];the Pearson correlation was used to analyze the correlation of serum miR-34a-5p and CTRP9 and their correlation with myocardial remodeling indexes respectively;the multivariate Logistic regression analysis was adopted to analyze the influencing factors of acute myocardial infarction occurrence in the patients.Results Compared with the control group,the level of serum miR-34a-5p,LVPW,LVMI and LAD in the observation group were significantly increased,while the level of CTRP9 and LVRI were significantly decreased(P<0.05);the Pearson correlation analysis results showed that miR-34a-5p was negatively correlated with CTRP9 and LVRI(r=-0.572,-0.645,P<0.05),and positively correlated with LVPW,LVMI and LAD(r=0.418,0.420,0.569,P<0.05);CTRP9 was positively correlated with LVRI(r=0.424,P<0.05),and negatively correlated with LVPW,LVMI and LAD(r=-0.583,-0.469,-0.518,P<0.05).The multivariate Logistic regression analysis results showed that miR-34a-5p level,LVPW,LVMI,LAD,LVRI and CTRP9 level all were the influencing factors or the occurrence of acute myocardial infarction(P<
