慢性心力衰竭患者血清miR-30a、miR-101a表达及其与心肌重构和心功能的相关性 被引量:19

Expression of serum miR-30a and miR-101a in patients with chronic heart failure and its correlation with myocardial remodeling and cardiac function

摘要 目的研究慢性心力衰竭(CHF)患者血清微RNA-30a(miR-30a)、miR-101a表达及其与心肌重构和心功能的相关性。方法选取2017年3月至2019年6月四川省雅安市人民医院收治的CHF患者128例为CHF组,参照纽约心脏病协会(NYHA)心功能分级标准分为:Ⅰ+Ⅱ级(n=41)、Ⅲ级(n=50)、Ⅳ级(n=37),另取同期门诊体检健康者50例为对照组。收集所有受试者血清,采用实时荧光定量PCR(RT-PCR)检测血清miR-30a、miR-101a mRNA水平,采用超声心动图检查所有受试者心功能指标:心输出量(CO)、左室射血分数(LVEF)、左室后壁厚度(LVPW)、左室舒张末期内径(LVEDD)、左房内径(LAD)、左室质量指数(LVMI)、左室重构指数(LVRI),采用Pearson法分析血清miR-30a、miR-101a mRNA水平与CHF患者CO、LVEF、LVPW、LVEDD、LAD、LVMI、LVRI的相关性。结果与对照组比较,CHF组患者血清miR-30a mRNA水平、LVPW、LVEDD、LAD、LVMI升高(均P<0.05),且随心功能分级增加而逐渐升高,miR-101a mRNA水平、CO、LVEF、LVRI降低(均P<0.05),且随心功能分级增加而逐渐降低。CHF患者中,血清miR-30a mRNA与miR-101a mRNA呈负相关;miR-30a mRNA与CO、LVEF、LVRI呈负相关,与LVPW、LAD、LVMI呈正相关(均P<0.05);miR-101a mRNA水平与CO、LVEF、LVRI呈正相关,与LVPW、LAD、LVMI呈负相关(均P<0.05),血清miR-30a mRNA、miR-101a mRNA水平与LVEDD无相关性(P>0.05)。结论血清miR-30a、miR-101a水平随CHF患者心功能分级增加分别升高和降低,与心肌重构和心功能密切相关,对CHF患者病情严重程度及预后评估有一定指导意义。 Objective To study the expression of serum microRNA-30a(miR-30a)and miR-101a in patients with chronic heart failure(CHF)and their correlation with myocardial remodeling and cardiac function.Methods A total of 128 CHF patients admitted to People′s Hospital of of Ya′an City from March 2017 to June 2019 were selected as the CHF group.According to the New York Heart Association(NYHA)cardiac function classification standard,they were divided into gradeⅠ+Ⅱ(n=41),gradeⅢ(n=50),gradeⅣ(n=37),and 50 healthy people who had physical examination in the same period were selected as the control group.The serum of all subjects was collected,real-time fluorescent PCR(RT-PCR)was used to detect the expression levels of serum miR-30a and miR-101a.Cardiac function indicators such as cardiac output(CO),left ventricular ejection fraction(LVEF),left ventricular posterior wall thickness(LVPW),left ventricular end diastolic diameter(LVEDD),left atrial diameter(LAD),left ventricular mass index(LVMI)and left ventricular remodeling index(LVRI)were measured by echocardiography in all subjects.Pearson method was used to analyze the correlations between levels of serum miR-30a mRNA,miR-101a mRNA and CO,LVEF,LVPW,LVEDD,LAD,LVMI,LVRI in CHF patients.Results Compared with the control group,the serum miR-30a level,LVPW,LVEDD,LAD,and LVMI of the CHF group were all increased,and they were gradually increased with the increase of the cardiac function classification.The miR-101a mRNA level,CO level,LVEF,LVRI were all decreased,and they were gradually decreased with the increase of the cardiac function classification(all P<0.05).In CHF patients,serum miR-30a level was negatively correlated with miR-101a mRNA level.The level of miR-30a mRNA was negatively correlated with CO,LVEF,and LVRI,and positively correlated with LVPW,LAD,and LVMI(all P<0.05);The level of miR-101a mRNA was positively correlated with CO,LVEF and LVRI in CHF patients,and negatively correlated with LVPW,LAD and LVMI(all P<0.05).The levels of serum miR-30a mRNA and miR-101a m
