摘要
目的:探究重型颅脑损伤(TBI)患者尿路感染病原菌分布及影响因素。方法:回顾性分析2020年1月—2022年9月浙江大学医学院附属邵逸夫医院收治的132例重型TBI患者的临床资料。收集所有患者一般资料。统计所有患者尿路感染、病原菌分布及分组情况。比较两组一般资料。分析重型TBI患者尿路感染影响因素。结果:132例患者中24例发生尿路感染,发生率为18.18%,纳入感染组。24例尿路感染患者中,共检出39株菌株,分别为:革兰阴性菌21株,革兰阳性菌7株,真菌11株。感染组住院时间、留置导尿管时间长于未感染组,开放性TBI、合并糖尿病、合并症≥2个占比均高于未感染组,入院格拉斯哥昏迷量表(GCS)评分低于未感染组,差异有统计学意义(P<0.05);合并糖尿病、入院GCS评分及留置导尿管时间均为重型TBI患者尿路感染发生的影响因素(P<0.05)。结论:重型TBI尿路感染患者主要以革兰阴性菌为主,合并糖尿病、入院GCS评分及留置导尿管时间是重型TBI患者发生尿路感染的影响因素。
Objective:To explore the pathogenic bacteria distribution and influencing factors of urinary tract infection in patients with traumatic brain injury(TBI).Method:The clinical data of 132 patients with severe TBI admitted to Sir Run Run Shaw Hospital,School of Medicine,Zhejiang University from January 2020 to September 2022 were retrospectively analyzed.General data of all patients were collected.The urinary tract infection condition and the distribution condition of pathogenic bacteria and grouping condition of all patients were analyzed.The general data of the two groups were compared.The influencing factors of urinary tract infection in severe TBI patients were analyzed.Result:Among 132 patients,24 happened urinary tract infection,the incidence was 18.18%and they were included in the infection group.In 24 patients with urinary tract infection,a total of 39 strains were detected,including 21 gram-negative bacteria,7 gram-positive bacteria and 11 fungi.The duration of hospital stay and indentation catheter in the infected group was longer than that in the uninfected group,the proportions of open TBI,diabetes mellitus,and complications≥2 were higher than those in the uninfected group,and the Glasgow coma scale(GCS)score at admission was lower than that in the uninfected group,the differences were statistically significant(P<0.05).Combined diabetes mellitus,GCS score at admission and indwelling catheter time were the influencing factors of urinary tract infection in severe TBI patients(P<0.05).Conclusion:Patients with severe TBI urinary tract infection are mainly dominated by gram-negative bacteria,and diabetes mellitus,GCS score at admission and indentation catheter time are influential factors for the occurrence of urinary tract infection in severe TBI patients.
