摘要
目的评定血常规联合C-反应蛋白(CRP)诊断儿童上呼吸道感染的效能和临床价值。方法选取2022年1—12月在枣庄市峄城区人民医院接受治疗的上呼吸道感染患儿185例,依据患儿病原体类型的不同,将其分成支原体组34例、细菌组64例和病毒组87例,另选取同期来该院进行健康体检的健康儿童80例,将其作为本次研究的对照组。测定并比较4组的血清CRP和白细胞计数(WBC)。结果细菌组血清CRP、WBC明显高于支原体组、病毒组、对照组,差异均有统计学意义(P<0.05)。支原体组、病毒组血清CRP高于对照组;病毒组WBC低于对照组和支原体组;支原体组WBC高于对照组,差异均有统计学意义(P<0.05)。WBC联合CRP诊断上呼吸道感染的准确度、灵敏度均高于WBC、CRP,特异度低于WBC、CRP,差异均有统计学意义(P<0.05)。结论WBC、CRP对感染病原体类型的鉴别有利,而将二者进行联合,能够提高诊断上呼吸道感染的准确度和灵敏度,值得临床推广应用。
Objective To evaluate the efficacy and clinical value of blood routine combined with C-reactive protein(CRP)in the diagnosis of upper respiratory tract infections in children.Methods A total of 185 children with upper respiratory tract infections who came to Zaozhuang City Yicheng District People's Hospital for treatment from January to December 2022 were selected and divided into the mycoplasma group with 34 cases,the bacterial group with 64 cases and the viral group with 87 cases based on the different types of pathogens in the children.In addition,80 healthy children who came to this hospital for health examinations during the same period were selected as the control group for this study.The serum CRP and white blood cell count(WBC)of four groups were measured and compared.Results The serum CRP and WBC in the bacterial group were significantly higher than those in the mycoplasma group,viral group and control group,and the differences were statistically significant(P<0.05).The serum CRP in the mycoplasma group and viral group were higher than those in the control group;the WBC in the viral group was lower than that in the control group and mycoplasma group,the WBC in the mycoplasma group was higher than that in the control group,and the differences were statistically significant(P<0.05).The accuracy and sensitivity of WBC combined with CRP in diagnosing upper respiratory tract infections were higher than those of WBC and CRP,the specificity was lower than that of WBC and CRP,and the differences were statistically significant(P<0.05).Conclusion WBC and CRP were beneficial for distinguishing the types of infectious pathogens,and combining them could improve the accuracy and sensitivity of diagnosing upper respiratory tract infections,which was worthy of clinical promotion and application.
