摘要
目的探讨红细胞平均体积(MCV)、血红蛋白(HGB)及红细胞体积分布宽度(RDW)在妊娠期地中海贫血患者中的表达及相关性。方法选择2019年4月—2022年4月乐山市市中区妇幼保健院经收治的500例妊娠期经地贫基因检测确诊为地中海贫血患者,为地贫组;同期选取在经血常规、贫血3项确诊为缺铁性贫血妊娠孕妇患者300例,为非地贫组,并且同期于乐山市市中区妇幼保健院门诊体检的800例正常孕妇为本研究的对照组。电化学发光免疫分析仪检测SF水平;全自动血细胞分析仪检测MCV、HGB、RDW及RBC水平,分析MCV、HGB及RDW水平在妊娠期地中海贫血患者中表达及关系。结果对照组MCV、HGB、RDW水平分别为(89.58±5.25)fl、(121.93±12.73)g/L、(11.14±1.26)%,地贫组MCV、HGB、RDW水平分别为(66.14±5.12)fl、(101.26±8.41)g/L、(14.19±2.49)%,与对照组相比,地贫组MCV、HGB水平降低,RDW水平升高,差异均有统计学意义(均P<0.05)。轻度妊娠期地中海贫血患者MCV、HGB及RDW水平分别为(69.55±5.34)fl、(108.45±8.76)g/L、(12.03±1.04)%,中度妊娠期地中海贫血患者MCV、HGB及RDW水平分别为(58.75±4.16)fl、(96.34±7.34)g/L、(15.44±2.73)%,重度妊娠期地中海贫血患者MCV、HGB及RDW水平分别为(49.21±3.49)fl、(87.49±6.82)g/L、(20.26±3.55)%,与轻度、中度妊娠期地中海贫血患者相比,重度妊娠期地中海贫血患者MCV、HGB水平降低幅度最大,RDW水平上升幅度最大,差异均有统计学意义(均P<0.05)。妊娠期地中海贫血中MCV、HGB表达呈正相关(r=0.383,P<0.05);HGB、RDW表达呈负相关(r=-0.451,P<0.05);MCV、RDW表达呈负相关(r=-0.387,P<0.05)。受试者特征曲线(ROC)分析MCV、HGB、RDW及联合检测对妊娠期地中海贫血患者的诊断价值,其曲下面积分别为0.777、0.758及0.732。结论妊娠期地中海贫血患者MCV、HGB水平显著降低,RDW水平显著升高,低水平MCV、HGB,高水平RDW是妊娠期地中海贫血患者的风险预�
Objective To explore the expression and correlation of mean red blood cell volume(MCV),hemoglobin(HGB)and red blood cell volume distribution width(RDW)in patients with thalassemia during pregnancy.Methods A total of 500 pregnant patients with thalassemia confirmed by thalassemia gene testing inLeshan City Shizhong District Maternal and Child Health Hospital from April 2019 to April 2022 were selected as thalassemia group.During the same period,300 pregnant women diagnosed with iron deficiency anemia were selected in the 3 items of menstrual blood routine and anemia,and they were in the non-thalassemia group.In the same period,800 normal pregnant women who underwent physical examination in Leshan City Shizhong District Maternal and Child Health Hospital outpatient physical examination were selected as the control group of this study.Electrochemiluminescence immunoassay was used to detect the level of SF;automatic blood cell analyzer was used to detect the levels of MCV,HCB,RDW,and RBC,and to analyze the expression and relationship of MCV,HGB,and RDW levels in patients with thalassemia during pregnancy.Results The levels of MCV,HGB and RDW in the control group were(89.58±5.25)fl,(121.93±12.73)g/L and(11.14±1.26)%,respectively.The levels of MCV,HGB and RDW in the thalassemia group were(66.14±5.12)fl,(101.26±8.41)g/L and(14.19±2.49)%respectively.Compared with the control group,the levels of MCV and HCB in the thalassemia group were decreased,and the level of RDW was increased,and the difference was statistically significant(P<0.05).The levels of MCV,HGB and RDW in the thalassemia patients with mild pregnancy were(69.55±5.34)fl,(108.45±8.76)g/L and(12.03±1.04)%,respectively.The levels of MCV,HCB and RDW in the thalassemia patients with moderate pregnancy were(58.75±4.16)fl,(96.34±7.34)g/L and(15.44±2.73)%,respectively.The levels of MCV,HGB and RDW in the thalassemia patients with severe pregnancy were(49.21±3.49)fl,(87.49±6.82)g/L and(20.26±3.55)%,respectively.Compared with the control group,the levels
