摘要
盆腔炎性疾病后遗症(SPID)是以女性内生殖器及周围组织发生炎症病变为特点的慢性感染性疾病,其发病率居高不下,迁延难愈,严重威胁女性身心健康。目前,西医治疗该病以抗菌消炎为主,但易出现药物敏感性降低、不良反应大等问题。中药及其复方制剂在改善SPID患者临床症状、降低不良反应等方面优势显著,在辨证论治和整体观念理论指导下,调整机体气血阴阳,应用前景广阔。桂枝茯苓方作为化瘀生新、缓消癥块的经典名方,由汉代张仲景所创,首载于《金匮要略》,临床上被广泛应用于SPID的治疗。目前机制研究表明,桂枝茯苓方通过多种途径对SPID发挥作用,包括减轻炎症反应、调节机体免疫功能、改善氧化应激损伤、改善血液流变学和盆腔血流动力学以及抑制组织纤维化等。临床研究显示,桂枝茯苓方能显著改善由SPID导致的慢性盆腔痛等症状,并有效提高患者的机体免疫功能、改善中医证候积分、改善输卵管通畅度和促进胚胎着床,增加怀孕概率,且不良反应少,安全性较高。文章从方证基础、机制与临床研究等方面对桂枝茯苓方治疗SPID进行归纳总结,以期为日后该病的治疗提供准确、系统的科学依据,为临床用药提供参考。
The sequelae of pelvic inflammatory disease(SPID)is a chronic infectious disorders characterized by inflammation of the female internal genital organs and surrounding tissues.The global incidence of this disease remains high,causing prolonged illnesses that stretch out over months and pose a serious threat to women's physical and mental health.At present,the primary treatment approach in Western medicine involves the administration of antibiotics and anti-inflammatory drugs.However,these therapeutic methods may lead to a series of side effects,reduced drug sensitivity,and other complications.In patients with SPID,traditional Chinese medicine,or Decoction,has been demonstrated to have significant advantages in improving clinical symptoms and lowering toxicities.As it may regulate the body's Yin and Yang,Qi and blood based on syndrome differentiation,treatment,and overall concepts,it has a wide range of potential applications.Guizhi Fuling Decoction(桂枝茯苓方),a classic formula created by ZHANG Zhongjing during the Han Dynasty with the aim of resolving stasis,generating new blood,and eliminating masses,has been extensively utilized in SPID since its initial publication in the Synopsis of Golden Chamber.The current mechanistic studies have demonstrated that Guizhi Fuling Decoction exerts its effects on SPID through multiple mechanisms,including the attenuation of inflammatory response,modulation of immune function,amelioration of oxidative stress-induced damage,enhancement of hemorheology and pelvic hemodynamics,as well as inhibition of tissue fibrosis.Clinical studies have shown that Guizhi Fuling Decoction effectively alleviates symptoms of chronic pelvic pain caused by SPID.Additionally,it boosts immune function,improves traditional Chinese medicine syndrome scores,enhances fallopian tube patency,promotes embryo implantation,increases the likelihood of pregnancy,and exhibits minimal adverse reactions with a high level of safety.This article provides a comprehensive and systematic scientific basis for the fu
