常规超声联合超声造影对长径≤2.0 cm乳腺肿块良恶性的鉴别诊断价值

Value of conventional ultrasound combined with contrast-enhanced ultrasound in differential diagnosis of benign and malignant breast masses≤2.0 cm in major diameter

摘要 目的探究常规超声联合超声造影对长径≤2.0 cm乳腺肿块良恶性的鉴别诊断价值。方法回顾性分析2022年2月至2023年11月铜川市人民医院收治的96例乳腺肿块患者的临床资料,根据手术或病理活检结果将其分为良性组(46例)与恶性组(50例)。比较两组常规超声、超声造影声像图特征及相应检查定量参数。以手术或病理活检结果为"金标准",比较并分析常规超声、超声造影单独及二者联合诊断的结果和诊断效能。结果恶性组边界模糊、无包膜、微钙化、后方回声衰减、内部回声不均匀、血流丰富、增强不均匀、病灶显著增大、穿入或扭曲血管比例高于良性组(P<0.05)。良性组峰值强度、曲线上升支斜率高于恶性组,动脉血流阻力指数、最大血流速度低于恶性组(P<0.05);两组达峰时间值比较差异未见统计学意义(P>0.05)。常规超声联合超声造影的灵敏度(96.00%,48/50)、阴性预测值(94.87%,37/39)均高于常规超声(70.00%,35/50;71.17%,37/52)及超声造影单独诊断(78.00%,39/50;77.55%,38/49),常规超声联合超声造影的漏诊率(4.00%,2/50)低于常规超声(30.00%,15/50)及超声造影单独诊断(22.00%,11/50),差异有统计学意义(P<0.05)。结论常规超声、超声造影在长径≤2.0 cm乳腺肿块良恶性的定性诊断中具有一定鉴别诊断价值,两者联合可显著提升在长径≤2.0 cm乳腺肿块中的鉴别诊断价值。 Objective To investigate the value of conventional ultrasound combined with contrast-enhanced ultrasound in the differential diagnosis of breast mass with major diameter≤2.0 cm.Methods The clinical data of 96 patients with breast mass admitted to Tongchuan People’s Hospital from February 2022 to November 2023 were retrospectively analyzed.And they were divided into a benign group(46 cases)and a malignant group(50 cases)according to surgical or pathological biopsy results.The characteristics and quantitative parameters of conventional ultrasound and contrast-enhanced ultrasound images were compared between the two groups.Taking the results of surgical or pathological biopsy results as the gold standard,the diagnostic results and efficacy of conventional ultrasound alone,contrast-enhanced ultrasound alone and combination of them were compared.Results The proportions of patients with boundary ambiguity,absence of envelope,microcalcification,attenuation of posterior echo,uneven internal echo,abundant blood flow,uneven enhancement,significant enlargement of lesion,penetration or distortion of blood vessel in the malignant group were higher than those in the benign group(P<0.05).The peak intensity and slope of the ascending branch of the curve in the benign group were significantly higher than those in the malignant group,moreover,the arterial blood flow resistance index(RI)and maximum blood flow velocity in the benign group were lower than those in the malignant group(P<0.05).There was no significant difference in peak time between the two groups(P>0.05).The sensitivity and negative predictive value of conventional ultrasound combined with contrast-enhanced ultrasound were 96.00%(48/50)and 94.87%(37/39),higher than those of conventional ultrasound alone(70.00%,35/50;71.17%,37/52)and contrast-enhanced ultrasound alone(78.00%,39/50;77.55%,38/49),while the missed diagnosis rate of combined diagnosis was 4.00%(2/50),lower than that of conventional ultrasound alone(30.00%,15/50)and contrast-enhanced ultrasound alone(22
