摘要
目的探讨超声造影(CEUS)技术联合应用乳腺影像报告和数据系统(BI-RADS)分类,在RADS 4类乳腺肿块中的诊断价值。方法 2015年11月—2016年5月于河北省人民医院诊治的60例BI-RADS 4类乳腺肿块患者,均行超声造影检查,对造影表现进行综合评分,以病理结果作为金标准,分别评估单独应用CEUS、BI-RADS分类及二者联合应用的诊断效能。结果 60个乳腺病灶中,良性病灶34个,恶性病灶26个。良性组病灶与恶性组病灶在增强强度、边界、造影剂分布形式、充盈缺损、周边放射状增强比较,差异均具有统计学意义(P<0.05)。恶性组织造影前后面积比(AR)、时间—强度曲线(PT)高于良性组(t=4.20、3.31,P<0.05)。超声造影评分的诊断符合率为80.0%,BI-RADS分类的诊断符合率为78.3%,二者联合应用的诊断符合率(90.0%),高于二者单独应用。结论BI-RADS分类联合超声造影技术比单独应用BI-RADS对乳腺不典型肿块的诊断准确性更高。
Objective To explore the diagnostic efficacy of contrast enhanced ultrasound（CEUS）in breast lesions classified BI-RADS 4 by combined with BI-RADS classifying.Methods A total of 60 lesions classified BI-RADS 4 were ex-amined by CEUS before surgery.According to the pathological result, the efficacy of BI-RADS classifying, CEUS grade and combining both methods were evaluated.Results Of the 60 mammary lesions,34 benign lesions and 26 malignant lesions. The lesion of benign group and malignant group was statistically significant（P〈0.05）in intensity,boundary,contrast agent distribution,filling defect and peripheral radial enhancement.The area ratio（AR）,time-intensity curve（PT）was higher in malignant group than the benign group（t=4.20,t=3.31, P〈0.05）.The diagnostic compliance rate of ultrasonography score was 80.0%,and the diagnostic compliance rate of BI-RADS classification was 78.3%.The diagnostic compliance rate of the combined application was 90.0%,which was higher than that of separate use of either method.Conclusion The com-bine of CEUS and BI-RADS has higher diagnostic efficacy in untypical lesions than separate use of either method.
