老年综合评估联合多维度健康教育在老年冠心病患者中的应用

Application of Geriatric Comprehensive Assessment Combined With Multi-Dimensional Health Education to Elderly Patients With Coronary Heart Disease

摘要 目的探讨老年综合评估(geriatric comprehensive assessment,GCA)联合多维度健康教育在老年冠心病患者中的应用。方法选取2021年12月—2023年6月山东第一医科大学附属省立医院老年心血管科收治的82例老年冠心病患者。随机分为对照组与试验组,各41例。对照组为常规治疗及护理,试验组在对照组基础上实施GCA联合多维度健康教育。干预前、干预12周后采用Morisky药物依从性量表(8-item Morisky medication adherence scale,MMAS-8)、冠心病自我管理行为量表(coronary artery disease self-management scale,CSMS)、中国心血管患者生活质量评定问卷(China questionnaire of quality life in patients with cardiovascular disease,CQQC)分别评估服药依从性、自我管理能力、生活质量。结果干预12周后,试验组用药依从性、日常生活管理、疾病医学管理、情绪管理评分及总分分别为(7.02±0.90)分、(32.08±6.20)分、(47.80±9.98)分、(12.83±3.68)分、(92.70±13.13)分,均高于对照组的(5.01±1.64)分、(23.15±7.19)分、(40.80±11.99)分、(10.85±5.02)分、(74.80±15.96)分(P<0.05);干预12周后,试验组生活质量中病情、一般生活、社会心理维度及总分高于对照组(P<0.05)。结论GCA联合多维度健康教育可提高老年冠心病患者服药依从性及自我管理能力,从而改善其生活质量。 Objective To explore the application of geriatric comprehensive assessment(GCA)combined with multidimensional health education in elderly patients with coronary heart disease.Methods A total of 82 elderly patients with coronary heart disease admitted to department of geriatric cardiovascular,Shandong Provincial Hospital Affiliated to Shandong First Medical University from December 2021 to June 2023 were selected.Randomly divided into control group and experimental group,41 cases in each group.The control group received conventional treatment and nursing,and the experimental group received GCA combined with multi-dimensional health education based on the control group.8-Item Morisky medication adherence scale(MMAS-8),coronary artery disease self-management scale(CSMS)and China questionnaire of quality life in patients with cardiovascular disease(CQQC)were used to evaluate medication compliance,self-management ability and quality of life before intervention and 12 weeks after intervention,respectively.Results After 12 weeks of intervention,the scores of medication compliance,daily life management,medical disease management and emotional management and total scores of the experimental groups were(7.02±0.90)points,(32.08±6.20)points,(47.80±9.98)points,(12.83±3.68)points and(92.70±13.13)points,respectively,they were higher than(5.01±1.64)points,(23.15±7.19)points,(40.80±11.99)points,(10.85±5.02)points,(74.80±15.96)points in the control group(P<0.05).After 12 weeks of intervention,the quality of life of the experimental group was higher than that of the control group in terms of illness,general life,psychosocial dimension and total score(P<0.05).Conclusion GCA combined with multi-dimensional health education can improve the medication compliance and self-management ability of elderly patients with coronary heart disease,so as to improve their quality of life.
