摘要
目的探讨子宫肌瘤(UM)患者在腹腔镜子宫肌瘤剔除术后增加中医二联法(中药直肠滴入联合针灸)治疗的临床效果。方法从南昌市洪都中医院2022年1月—2022年9月收治的UM患者中抽取60例作为研究对象,使用随机数字表法分为试验组和对照组,每组30例。对照组和试验组采用的治疗方法分别为腹腔镜子宫肌瘤剔除术、腹腔镜子宫肌瘤剔除术加用中医二联法,对比两组的复发率、妊娠率、临床症状改善时间、治疗前后的性激素水平和卵巢储备功能。结果试验组的妊娠率93.33%(28/30)高于对照组的73.33%(22/30)(P<0.05);试验组的复发率3.33%(1/30)低于对照组的26.67%(8/30)(P<0.05);试验组下腹坠胀感消失时间、贫血症状改善时间、月经转归时间和阴道异常出血消失时间均较对照组短(P<0.05)。治疗后试验组的雌二醇(E_(2))、促黄体生成素(LH)和促卵泡生成素(FSH)水平均较对照组低(P<0.05),抗缪勒管激素(AMH)水平较对照组高(P<0.05)。结论在腹腔镜子宫肌瘤剔除术后增加中药直肠滴入联合针灸治疗,不仅能提高UM患者妊娠率,降低复发率,改善其临床症状,还能调节其性激素水平和AMH水平,临床可进一步推广。
Objective To investigate the clinical effect of Chinese medicine rectal drip combined with acupuncture and moxibustion in the treatment of uterine myoma(UM)during laparoscopic myomectomy.Methods A total of 60 UM patients admitted to Nanchang Hongdu Hospital of Traditional Chinese Medicine from January 2022 to September 2022 were selected as research objects,and were divided into experimental group and control group using random number table method,with 30 cases in each group.The treatment methods used in the control group and experimental group were laparoscopic myomectomy,laparoscopic myomectomy combined with Chinese medicine treatment,respectively.The recurrence rate,pregnancy rate,clinical symptom improvement time,sex hormone lev⁃els and ovarian reserve function(AMH)before and after treatment were compared between the two groups.Results The pregnancy rate of experimental group was 93.33%(28/30),which was higher than that of control group(73.33%,22/30)(P<0.05).The recurrence rate of experimental group was 3.33%(1/30),which was lower than that of control group(26.67%,8/30)(P<0.05).The disappearance time of lower abdominal distension,the improvement time of anemia symptoms,the outcome time of menstruation and the disappearance time of abnormal vaginal bleeding in experimental group were shorter than those in control group(P<0.05).After treatment,the levels of es⁃tradiol(E_(2)),luteinizing hormone(LH)and follicle-stimulating hormone(FSH)in experimental group were lower than those in control group(P<0.05),and the levels of anti-Muller hormone(AMH)in experimental group were higher than those in control group(P<0.05).Conclusion The treatment of UM patients with Chinese medicine rectal drip combined with acupuncture and moxibustion in the treat⁃ment of can not only improve their pregnancy rate,reduce the recurrence rate,improve their clinical symptoms,but also regulate their sex hormone levels and AMH levels,which can be further promoted in clinic.
