摘要
目的:探讨循经往返灸联合推拿在脑卒中痉挛性偏瘫(SHAS)中的应用效果。方法:队列研究。抽取2022年5月至2023年5月北京中医药大学孙思邈医院收治的200例SHAS患者,按随机数字表法分为对照组和观察组,每组100例。对照组予以康复训练治疗,观察组加用循经往返灸联合推拿治疗,持续4周。比较两组临床疗效、肢体痉挛程度、运动功能、中医症状积分。结果:观察组总有效率(96.00%,96/100)高于对照组(87.00%,87/100),P<0.05。治疗后,观察组临床痉挛指数评分[(6.15±1.13)分]低于对照组[(8.53±1.25)分],Fugl-Meyer运动功能测评法评分[(75.56±5.39)分]高于对照组[(67.44±5.25)],P<0.05。治疗后,观察组半身不遂、口舌歪斜、手足拘急、肢体麻木积分均低于对照组(P均<0.05)。结论:循经往返灸联合推拿可提高SHAS患者的康复效果,减轻肢体痉挛程度,加快运动功能恢复。
Objective:To investigate the application effect of round-trip moxibustion along the meridian combined with massage in the treatment of spastic hemiplegia after stroke(SHAS).Methods:A total of 200 SHAS patients admitted to Sun Simiao Hospital of Beijing University of Traditional Chinese Medicine from May 2022 to May 2023 were selected for the cohort study.And they were divided into a control group and an observation group by random number table method,with 100 cases in each group.The control group was treated by rehabilitation training,in addition,the observation group was treated by round-trip moxibustion along the meridian combined with massage for 4 weeks continuously.The clinical efficacy,degree of limb spasms,motor function,traditional Chinese medicine symptom scores of the two groups were compared.Results:The total effective rate of the observation group(96.00%,96/100)was higher than that of the control group(87.00%,87/100),P<0.05.After treatment,the clinic spasticity index of the observation group(6.15±1.13)was lower than that of the control group(8.53±1.25),while the Fugl-Meyer motor function assessment score of the observation group(75.56±5.39)was higher than that of the control group(67.44±5.25),P<0.05.After treatment,the observation group had lower scores for hemiplegia,tongue and mouth deviation,hand and foot stiffness,and limb numbness compared with the control group(all P<0.05).Conclusions:Round-trip moxibustion along the meridian combined with massage can improve the rehabilitation effect of SHAS,ameliorate limb spasms,and accelerate motor function recovery.
