摘要
目的 探讨瘙痒和睡眠质量对成人中重度特应性皮炎(AD)患者的生活质量的影响,提高目前疾病管理和治疗策略。方法 采用便利抽样法,选取2023年1~12月于上海市皮肤病医院AD诊疗中心收治的244例成人中重度AD患者作为研究对象,采用一般资料调查表、皮肤病生活质量指数(DLQI)、AD控制工具(ADCT)、瘙痒数字分级法(NRS)评分和睡眠视觉模拟评分(VAS)问卷进行横断面调查。根据DLQI评分是否>5分,将患者分为生活质量受损组和生活质量未受损组。采用多因素逻辑回归方法分析探究成人中重度AD患者生活质量受损的影响因素。通过限制性立方样条图分析,进一步探讨瘙痒程度和睡眠质量与成人中重度AD患者生活质量受损之间的关系。结果 本研究纳入了244例成人中重度AD患者,男性117例,女性127例,其中192例患者存在生活质量受损(DLQI评分>5分)。与生活质量未受损组患者比较,生活质量受损组患者ADCT评分(P<0.001)、瘙痒数字分级法(NRS)评分(P<0.001)和睡眠视觉模拟评分(VAS)均明显升高(P=0.001)。Logistic回归分析结果显示瘙痒程度和睡眠质量是成人中重度AD患者生活质量受损的独立危险因素。此外,瘙痒NRS和睡眠VAS与成人中重度AD患者生活质量受损存在显著的线性关系。结论 瘙痒和睡眠质量是影响成人中重度AD患者生活质量的重要影响因素,对瘙痒和睡眠质量的关注不仅有助于提高AD患者的整体生活质量,也有利于AD患者的长期管理。
Objective In order to investigate the influence of itch and sleep quality on the quality of life of adults with moderate-to-severe atopic dermatitis(AD),with the goal of enhancing current disease management and treatment strategies.Methods A total of 244 adults with severe AD treated at the AD clinic of the Skin Disease Hospital in Shanghai from January 2023 to December 2023 were conveniently sampled for a cross-sectional study.General information questionnaire,dermatology life quality index(DLQI),AD control tool(ADCT),itch numerical rating scale(NRS),and sleep visual analog scale(VAS)questionnaires were utilized.Multifactorial logistic regression analysis was employed to investigate factors influencing the impaired quality of life in adults with severe AD.Restricted cubic spline analysis was conducted to further explore the relationship between itch severity,sleep quality,and impaired quality of life in adults with severe AD.Results A total of 244 adults with severe AD were included in the study,with 117 males and 127 females,of which 192 patients experienced impaired quality of life(DLQI score>5).Compared to the group with unimpaired quality of life,patients in the impaired quality of life group had significantly higher ADCT scores(P<0.001),itch NRS scores(P<0.001),and sleep VAS scores(P=0.001).Logistic regression analysis results indicated that itch severity and sleep quality were independent risk factors for impaired quality of life in adults with severe AD.Additionally,there was a significant linear relationship among itch severity,sleep quality scores,and impaired quality of life in adults with severe AD.Conclusion Itchiness and sleep quality are important fators affecting the quality of life of adults with moderate-to-severe AD.Paying attention to itchiness and sleep quality not only helps to improve the overall quality of life of AD patients but also contributes to the long-term management of AD patients.
