摘要
目的:探究正骨推拿联合温阳通络针灸治疗腰椎间盘突出症(LDH)的效果。方法:选取2021年3月—2023年3月荆州市中医医院收治的106例LDH患者作为研究对象,应用随机数表法将其分为对照组(n=53)及研究组(n=53),对照组采用正骨推拿干预,研究组采用联合温阳通络针灸干预。对比两组临床疗效、日本骨科学会(JOA)评分、Oswestry功能障碍指数(ODI)、疼痛视觉模拟评分(VAS)、炎症因子[白细胞介素-17(IL-17)、白细胞介素-22(IL-22)、白细胞介素-27(IL-27)]、致痛因子[前列腺素-E2(PGE2)、5-羟色胺(5-HT)、磷脂酶A2(PLA2)]情况。结果:研究组患者临床疗效优于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者JOA升高、ODI、VAS降低,研究组患者JOA高于对照组,ODI、VAS低于对照组,差异有统计学意义(P<0.05);治疗后,两组患者IL-17、IL-22、IL-27均降低,研究组低于对照组,差异有统计学意义(P<0.05);治疗后,两组患者PGE2、5-HT、PLA2均降低,研究组低于对照组,差异有统计学意义(P<0.05)。结论:LDH患者接受正骨推拿联合温阳通络针灸治疗,可改善致痛功能障碍,抑制疼痛因子、炎症因子释放,减轻疼痛,促进腰椎功能恢复。
Objective:To investigate the effect of orthopedic bone massage combined with Wenyang tongluo acupuncture quality lumbar disc herniation(LDH).Method:106 patients with LDH admitted to the hospital from March 2021 to March 2023 were divided into a control group and a research group by applying the random number table method,with 53 cases in each group.The patients of control group were treated with massage for bone orthopedics,while the patients of research group were treated with orthopedic bone massage combined with Wenyang tongluo acupuncture.The clinical efficacy,the score of the Japanese Orthopedic Association(JOA),the Oswestry Dysfunction Index(ODI),the visual analogue score of pain(VAS)Inflammatory factors[interleukin-17(IL-17),interleukin-22(IL-22),interleukin-27(IL-27)],and nociceptive factors[prostaglandin-E2(PGE2),5-hydroxytryptophan(5-HT),phospholipase A2(PLA2)]were compared between the two groups.Result:The clinical efficacy of the study group was better than that of the control group(P<0.05).After treatment,JOA increased,ODI and VAS decreased in both groups,and JIA was higher,ODI and VAS were lower in the study group than those in the control group(P<0.05).After treatment,IL-17,IL-22 and IL-27 were reduced in both groups,and the study group was lower than the control group(P<0.05).After treatment,PGE2,5-HT and PLA2 were reducing in both groups,and the study group was lower than the control group(P<0.05).Conclusion:LDH patients receiving orthopaedic massage combined with Wenyang tongluo acupuncture treatment can improve the pain-causing dysfunction,inhibit the release of pain factors and inflammatory factors,alleviate pain,and promote the recovery of lumbar spine function.
