摘要
目的探讨通络祛痛膏联合腰椎牵引治疗寒湿痹阻证腰椎间盘突出症(LDH)的效果观察。方法选取2020年2月至2022年3月在我院确诊的LDH患者120例,采用随机数表法分为观察组和对照组各60例。对照组采用常规基础治疗(双氯芬酸钠缓释片+维生素B1+维生素B6)联合腰椎牵引疗法,观察组在对照组的基础上联合通络祛痛膏治疗,两组均持续治疗15 d。比较两组治疗前后的临床疗效和中医证候积分,采用VAS评分、ODI指数和JOA量表评估患者治疗前后的腰部疼痛程度和腰椎活动功能,采用BI指数评估患者日常生活能力;比较两组患者治疗前后炎症因子及血管内皮细胞生长因子(VEGF)水平;观察两组治疗期间不良反应发生情况。结果治疗后,观察组腰背刺痛、坐骨神经痛等中医证候积分均降低,且观察组积分低于对照组(P<0.05);治疗15 d后,两组VAS和ODI评分均降低,且观察组评分低于对照组(P<0.05);两组JOA评分均升高,且观察组评分高于对照组(P<0.05);观察组BI指数、腰椎前屈和后伸程度评分均优于对照组(P<0.05);两组炎症因子水平均较治疗前降低,且观察组低于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义(P<0.05)。结论通络祛痛膏联合腰椎牵引治疗能够缓解寒湿痹阻证LDH患者疼痛,改善其活动能力,促进腰椎功能恢复。
Objective To explore the curative effect of Tong-luo-qu-tong ointment combined with lumbar traction in the treatment of patients with lumbar disc herniation(LDH)of cold-dampness obstruction syndrome.Methods A total of 120 patients with LDH confirmed in our hospital were enrolled between February 2020 and March 2022.According to random number table method,they were divided into an observation group and a control group,60 in each group.The control group was given routine basic treatments(diclofenac sodium sustained release tablets+vitamin B1+vitamin B6)combined with lumbar traction.The observation group was treated with Tong-luo-qu-tong ointment on basis of control group.All patients were continuously treated for 15 days.Clinical curative effect and scores of TCM syndromes before and after treatment were compared between the two groups.The lumbar pain and lumbar function before and after treatment were evaluated by VAS score,ODI index and JOA scale.The activities of daily living were evaluated by BI.The levels of inflammatory factors and vascular endothelial cell growth factor(VEGF)before and after treatment were compared between the two groups.The occurrence of adverse reactions in both groups during treatment was observed.Results After treatment,scores of TCM syndromes such as low back stabbing pain and sciatica were decreased in both groups and the scores were lower in the observation group than those in the control group(P<0.05).After 15 days of treatment,VAS and ODI scores in both groups were decreased,and the scores were lower in the observation group than those in the control group(P<0.05).After 15 days of treatment,JOA scores in both groups were increased,and the scores were higher in the observation group than those in the control group(P<0.05).After treatment,scores of BI,lumbar anteflexion and rear protraction in the observation group were better than those in the control group(P<0.05).After treatment,levels of inflammatory factors in both groups were decreased,and the levels were lower in the observatio
