血管生成拟态、Cripto-1在非小细胞肺癌组织中的表达及其临床意义

摘要 目的筛查预测非小细胞肺癌(NSCLC)侵袭、转移及预后的生物因子。方法采用免疫组织化学方法检测145例NSCLC患者血管生成拟态(vasculogenic mimicry,VM)的形成和Cripto-1的表达水平。结果Cripto-1在NSCLC中的表达率(56.6%)显著高于正常组织(36.6%)(P<0.001)。37.2%的NSCLC组织中存在VM,但在正常肺组织中未发现VM(P<0.05)。Cripto-1表达与VM形成显著相关(P<0.001)。高水平的Cripto-1和VM形成会导致总生存期(overall survival,OS)降低(P<0.001)。多因素分析表明,Cripto-1上调、VM形成、淋巴结转移和TNM分期是反映NSCLC预后的重要指标。结论Cripto-1和VM水平在NSCLC中显著升高,可能作为免疫标志物来预测该肿瘤的侵袭、转移和预后。 Objective To search for biological factors that might predict invasion,metastasis and prognosis of non-small cell lung cancer(NSCLC).Methods The immunohistochemical method was used to detect the formation of vasculogenic mimicry(VM)and the expression level of Cripto-1 in 145 cases of NSCLC.Results Cripto-1 levels were observably higher in the NSCLC tissues(56.6%)than in the healthy lung tissues(36.6%,P<0.001).VM was present only in the NSCLC tissues(37.2%vs 0,P<0.05).Cripto-1 expression was positively correlated with VM formation(P<0.001).Patients with high levels of Cripto-1 and VM formation had decreased overall survival(OS)(P<0.001).Moreover,multivariate analysis demonstrated that Cripto-1 upregulation,VM formation,TNM stage,and LNM were independent correlative factors affecting NSCLC prognosis.Conclusion Cripto-1 and VM levels are significantly elevated in NSCLC tissues and may serve as immune markers to predict invasion,metastasis,and prognosis in this cancer.
