多模态影像-超声弹性成像联合弥散加权成像对腮腺肿瘤良恶性的鉴别诊断价值

Value of multimodal imaging ultrasonic elastography combined with DWI in differential diagnosis of benign and malignant parotid gland tumors

摘要 目的分析多模态影像-超声弹性成像联合磁共振弥散加权成像(DWI)在腮腺肿瘤良恶性鉴别诊断中的价值。方法回顾性分析经手术或穿刺活检病理证实的68例腮腺肿瘤患者共74枚肿块的影像学资料。术前均行超声弹性成像及DWI检查,超声弹性成像采用5分法诊断,DWI采用测量病灶表观扩散系数进行诊断,两种诊断结果分别与病理结果进行比较。结果超声弹性成像诊断为恶性肿瘤24枚,包括真阳性16枚和假阳性8枚;诊断为良性肿瘤50枚,包括真阴性37枚和假阴性13枚;其诊断的灵敏度为55.2%,特异度为82.2%,准确度为71.6%。超声弹性成像联合DWI诊断为恶性肿瘤23枚,包括真阳性19枚和假阳性4枚;诊断为良性肿瘤51枚,包括真阴性46枚和假阴性5枚;其诊断的灵敏度为79.2%,特异度为92.0%,准确度为87.8%。超声弹性成像联合DWI对腮腺肿瘤良恶性诊断的准确度高于单用超声弹性成像(P<0.05)。结论与单用超声弹性成像相比,超声弹性成像联合DWI能提高腮腺肿瘤良恶性鉴别诊断的准确度。 Objective To analyze the value of multimodal imaging-ultrasonic elastography combined with diffusion-weighted imaging(DWI)in the differential diagnosis of benign and malignant parotid gland tumors.Methods The imaging data of 68 patients with parotid gland tumors(74 masses)confirmed by surgery or biopsy were retrospectively analyzed.Ultrasonic elastography and DWI were performed before surgery.The 5-point method was used in ultrasonic elastography,and DWI diagnose was performed by measuring the apparent diffusion coefficient value of the lesion The two diagnostic results were compared with the pathological results.Results Ultrasound elastography diagnosed 24 malignant tumors(including 16 true positives and 8 false positives)and 50 benign tumors(including 37 true negatives and 13 false negatives),in which the diagnostic sensitivity was 55.2%,specificity was 82.2%,and accuracy was 71.6%.Ultrasound elastography combined with DWI diagnosed 23 malignant tumors(including 19 true positives and 4 false positives)and 51 benign tumors(including 46 true negatives and 5 false negatives),in which the diagnostic sensitivity was 79.2%,specificity was 92.0%,and accuracy was 87.8%.The accuracy of ultrasound elastography combined with DWI in diagnosing benign and malignant parotid gland tumors was higher than that of ultrasound elastography alone(P<0.05).Conclusion Compared with ultrasound elastography alone,combined use of ultrasound elastography and DWI can improve the accuracy of the differential diagnosis of benign and malignant parotid gland tumors.
