摘要
目的分析经颅多普勒发泡试验联合右心声学造影对青年偏头痛合并卵圆孔未闭的诊断价值。方法回顾性抽取2020年6月至2023年4月于焦作市人民医院因偏头痛就诊的青年患者118例作为研究对象,所有研究对象均行经颅多普勒发泡试验、右心声学造影。以食管超声心动图检查结果为"金标准",比较经颅多普勒发泡试验、右心声学造影单独和联合检查的诊断结果、诊断效能、预测价值。结果经食道超声心动图检查显示偏头痛合并卵圆孔未闭45例,单纯偏头痛73例;经颅多普勒发泡试验检查出偏头痛合并卵圆孔未闭38例,单纯偏头痛59例;右心声学造影检查出偏头痛合并卵圆孔未闭34例,单纯偏头痛65例;二者联合检查出偏头痛合并卵圆孔未闭44例,单纯偏头痛71例。经颅多普勒发泡试验联合右心声学造影检查灵敏度(97.78%,44/45)、特异度为(97.26%,71/73)、准确度(97.46%,115/118),均高于经颅多普勒发泡试验(84.44%,38/45;80.82%,59/73;82.20%,97/118)和右心声学造影(75.56%,34/45;89.04%,65/73;83.90%,99/118),差异有统计学意义(P<0.05);经颅多普勒发泡试验联合右心声学造影检查漏诊率(2.22%,1/45)、误诊率(2.74%,2/73)低于经颅多普勒发泡试验(15.56%,7/45;19.18%,14/73)和右心声学造影(24.44%,11/45;10.96%,8/73),差异有统计学意义(P<0.05)。经颅多普勒发泡试验联合右心声学造影检查阳性预测率(95.65%,44/46)、阴性预测率(98.61%,71/72)均高于经颅多普勒发泡试验(73.08%,38/52;89.39%,59/66)和右心声学造影(80.95%,34/42;85.53%,65/76),差异有统计学意义(P<0.05)。结论经颅多普勒发泡试验联合右心声学造影诊断青年偏头痛合并卵圆孔未闭具有良好的诊断价值,可提高诊断效能,为患者早期诊疗提供科学依据。
Objective To analyze the diagnostic value of transcranial Doppler bubble test combined with right heart sonography for young migraineurs with patent foramen ovale.Methods A total of 118 young patients with migraine who visited Jiaozuo People’s Hospital from June 2020 to April 2023 were retrospectively selected as the study subjects.All patients underwent transcranial Doppler bubble test and right heart sonography.Taking the results of transesophageal echocardiography as the gold standard,the diagnostic results,diagnostic efficacy,and predict value of transcranial Doppler bubble test alone,right heart sonography alone and combination of them were compared.Results Transesophageal echocardiography showed that 45 patients had migraine with patent foramen ovale,and 73 patients had simple migraine.The transcranial Doppler bubble test detected out 38 cases of migraine with patent foramen ovale and 59 cases of simple migraine.The right heart sonography detected out 34 cases of migraine with patent foramen ovale and 65 cases of simple migraine.The combined examination detected out 44 cases of migraine with patent foramen ovale and 71 cases of simple migraine.The sensitivity of transcranial Doppler bubble test combined with right heart sonography was 97.78%(44/45),the specificity was 97.26%(71/73),and the accuracy was 97.46%(115/118),which were higher than those of transcranial Doppler bubble test(84.44%38/45;80.82%,59/73;82.20%,97/118)and right heart sonography(75.56%,34/45;89.04%,65/73;83.90%,99/118),P<0.05.The missed diagnosis rate and the misdiagnosis rate of trancranial Doppler bubble tese combined with right heart sonography were 2.22%(1/45)and 2.74%(2/73),respectively,which were lower than those of transcranial Doppler bubble test(15.56%,7/45;19.18%,14/73)and right heart sonography(24.44%,11/45;10.96%,8/73),P<0.05.The positive predicative value and the negative predicative value of transcranial Doppler bubble test combined with right heart sonography were 95.65%(44/46)and 98.61%(71/72),respectively,which were high
