摘要
目的研究表皮移植术联合中药汤剂治疗稳定期白癜风疗效及对患者免疫功能和生活质量的影响。方法将商丘市中医院2022年1月至2023年12月65例稳定期白癜风患者根据治疗方式的差异分为对照组(33例)和研究组(32例),对照组给予表皮移植术治疗,研究组在对照组基础上联合中药汤剂(白驳汤)治疗,比较疗效,并观察治疗后患者免疫功能指标CD4^(+)、CD4^(+)/CD8^(+)、CD8^(+)的改善情况及皮肤病生活质量指数(dermatology life quality Index,DLQI)的变化。结果本组患者治疗后研究组总有效率为93.75%,高于对照组(75.76%)(P<0.05);研究组治疗4周后白斑面积为(2.41±0.64)cm^(2),小于对照组[(3.19±0.89)cm^(2)],研究组白斑色素积分高于对照组,DLQI[(9.74±3.02)分]低于对照组[(12.44±3.11)分](P<0.05);研究组治疗4周后CD4^(+)、CD4^(+)/CD8^(+)水平高于对照组,CD8^(+)水平低于对照组(P<0.05);治疗期间,两组不良反应差异无统计学意义(P>0.05)。结论表皮移植术联合中药汤剂可提高稳定期白癜风治疗效果,改善免疫功能,安全性高,值得推广。
Objective To study the effect of epidermal transplantation combined with Chinese herbal decoction on vitiligo at stable stage and the effect on immune function and quality of life of patients.Methods From January 2022 to December 2023,totally 65 patients with stable vitiligo in Shangqiu Hospital of Traditional Chinese Medicine were divided into control group(33 cases)and study group(32 cases)according to the differences in treatment methods.The control group received epidermal transplantation treatment,and the study group received traditional Chinese medicine decoction(Baibo decoction)treatment on the basis of the control group.After treatment,the improvement of immune function indexes CD4^(+),CD4^(+)/CD8^(+),CD8^(+)and the changes of dermatology life quality index(DLQI)were observed.Results After treatment,the total effective rate of the study group was 93.75%,higher than that of the control group(75.76%)(P<0.05).After 4 weeks of treatment,the leukoplakia area of the study group was(2.41±0.64)cm^(2),which was smaller than that of the control group[(3.19±0.89)cm^(2)],the leukoplakia pigment score of the study group was higher than that of the control group,and DLQI[(9.74±3.02)points]was lower than that of control group[(12.44±3.11)points](P<0.05);after 4 weeks of treatment,the levels of CD4^(+)and CD4^(+)/CD8^(+)in the study group were higher than those in the control group,and the levels of CD8^(+)were lower than those in the control group(P<0.05).During treatment,there was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Epidermal transplantation combined with traditional Chinese medicine decoction can improve the treatment effect of vitiligo in stable stage,improve immune function,and have high safety.
