摘要
目的观察紫冰疮愈膏外敷对混合痔外剥内扎术后创面的影响。方法将70例混合痔患者随机分为观察组和对照组,每组35例。术后两组患者均接受抗感染、止痛等对症治疗,观察组、对照组患者分别加用紫冰疮愈膏、肛泰软膏换药。术后1、4、7、14 d,分别检测两组患者血清血管内皮生长因子(vascular endothelial growth factor,VEGF)水平,采用视觉模拟量表(visual analogue scale,VAS)评估创面疼痛程度,观察创面分泌物、创缘水肿、肉芽组织生长情况和创面愈合情况。结果术后4、7、14 d,观察组患者血清VEGF水平均显著高于对照组(P<0.05);观察组患者创面疼痛VAS评分、创面分泌物评分、创面肉芽组织生长情况评分、创缘水肿评分、创面面积少于对照组(P>0.05)或显著少于对照组(P<0.05)。结论紫冰疮愈膏外敷可减轻混合痔外剥内扎术后创面疼痛,减少创面分泌物,减轻创缘水肿,促进肉芽组织生长和创面愈合。
Objective To investigate the effect of external application of Zibing Chuangyu Ointment on the wound after Milligan-Morgan hemorrhoidectomy for mixed hemorrhoids.Methods A total of 70 patients with mixed hemorrhoids were randomly divided into observation group and control group,with 35 patients in each group.The patients in both groups were given symptomatic treatment such as anti-infective therapy and pain-relieving therapy after surgery,and in addition,the patients in the observation group were given Zibing Chuangyu Ointment for dressing change,while those in the control group were given Gangtai Ointment.On days 1,4,7,and 14 after surgery,the serum level of vascular endothelial growth factor(VEGF)was measured;Visual Analogue Scale(VAS)was used to assess the degree of pain;the wound was observed in terms of wound exudates,wound edge edema,growth of granulation tissue,and wound healing conditions.Results On days 4,7,and 14 after surgery,the observation group had a significantly higher serum level of VEGF than the control group(P<0.05).Compared with the control group,the observation group had lower or significantly lower VAS pain score,wound exudate score,granulation tissue score,wound edge edema score,and wound area(P>0.05 or P<0.05).Conclusion External application of Zibing Chuangyu Ointment can alleviate wound pain,reduce wound exudates and wound edge edema,promote the growth of granulation tissue,and facilitate wound healing after Milligan-Morgan hemorrhoidectomy for mixed hemorrhoids.
