摘要
目的:探讨阴道灌洗1号方联合乳酸杆菌治疗宫颈高危型人乳头瘤病毒(HR-HPV)持续感染的临床效果及对阴道微生态的影响。方法:选择2021年3月—2023年3月暨南大学附属第六医院妇科门诊收治的119例宫颈HR-HPV持续感染患者作为研究对象。随机分为三组,研究组(n=44)采用阴道灌洗1号方联合阴道用乳杆菌活菌胶囊治疗;干扰素组(n=41)采用人干扰素α2b阴道泡腾片治疗;空白组(n=34)拒绝用药治疗。观察三组HR-HPV转阴率及宫颈细胞学变化,分析三组阴道微生态的情况。结果:研究组HR-HPV转阴率为75.61%,高于干扰素组的52.63%及空白组的14.71%,差异有统计学意义(P<0.05)。治疗后,研究组细胞学升级率为2.44%,低于空白组的32.35%,差异有统计学意义(P<0.05)。研究组细胞学降级率为43.90%,高于干扰素组、空白组的21.05%、5.88%,差异有统计学意义(P<0.05)。治疗后,研究组过氧化氢(H2O2)、白细胞酯酶(LE)阴性占比均高于干扰素组及空白组,β-葡萄糖醛酸苷酶(GUS)、凝固酶(GADP)阴性占比高于空白组,差异有统计学意义(P<0.05)。结论:阴道灌洗1号方联合乳酸杆菌可高效清除宫颈HR-HPV感染,改善阴道微生态环境,抑制宫颈组织学升级。
Objective:To investigate the clinical efficacy of Vaginal Lavage No.1 Formula combined with Lactobacillus in the treatment of persistent cervical high-risk human papillomavirus(HR-HPV)infection and effect of vaginal microecology.Method:A total of 119 patients with persistent cervical HR-HPV infection admitted to the Gynecological Clinic of the Sixth Affiliated Hospital of Jinan University from March 2021 to March 2023 were selected as the study objects.They were randomly divided into three groups;the study group(n=44)was treated with Vaginal Lavage No.1 Formula combined with Live Lactobacillus Capsules for Vaginal Use,and the interferon group(n=41)was treated with Human Interferonα2b Vaginal Effervescent Tablets;the blank group(n=34)refused medication.The negative conversion rate of HR-HPV and the cytological changes of cervix in the three groups were observed,and the vaginal microecology of the three groups was analyzed.Result:The negative conversion rate of HR-HPV in the study group was 75.61%,which were higher than 52.63%,14.71%in the interferon group and the blank group,and the differences were statistically significant(P<0.05).After treatment,the cytological upgrade rate of the study group was 2.44%,which was lower than 32.35%of the blank group,the difference was statistically significant(P<0.05);the cytological degradation rate of the study group was 43.90%,which were higher than 21.05%and 5.88%of the interferon group and the blank group,the differences were statistically significant(P<0.05).After treatment,the negative proportion of hydrogen peroxide(H2O2)and leucocyte esterase(LE)in study group were higher than those in interferon group and blank group,and the negative proportion ofβ-glucuronidase(GUS)and coagulase(GADP)were higher than those in blank group,the differences were statistically significant(P<0.05).Conclusion:Vaginal Lavage No.1 Formula combined with Lactobacillus can effectively clear cervical HR-HPV infection,improve vaginal microecological environment,and inhibit cervical histological es
