骨质疏松性椎体压缩性骨折经皮骨水泥椎体成形术:研究进展与争议

Percutaneous Bone Cement Vertebroplasty for Osteoporotic Vertebral Compression Fractures:Research Progress and Controversy

摘要 骨质疏松性椎体压缩性骨折经皮骨水泥椎体成形术已成为临床常用治疗方式,相关议题已经成为临床热点。本研究就骨水泥注射灌注量、骨水泥弥散体积与椎体生物力学性能指标变化的相关性,以及骨水泥渗漏诱导髓核细胞损伤加速椎间盘退变等因素进行综述。骨水泥弥散体积结合了骨水泥注射灌注量及椎体骨密度两种指标,与椎体生物力学性能指标及疗效有相关性,临床应针对不同骨密度,选择相应的骨水泥灌注量,以期最优恢复骨折椎体的强度及刚度,提供可靠的脊柱稳定性;预防术后邻近节段椎体退变加重、继发性骨折,减少骨水泥渗漏致椎间盘退变的并发症,实现生物力学效应及临床效果均满意的目标。 Percutaneous bone cement vertebroplasty for osteoporotic vertebral compression fractures has become a common clinical method,and related issues have become clinical hotspots.This study reviewed the correlation between the injection volume of bone cement,the diffusion volume of bone cement and the changes of vertebral biomechanical properties,as well as factors such as bone cement leakage induced nucleus pulposus cell damage and accelerated intervertebral disc degeneration.Bone cement dispersion volume combines the two indicators of bone cement injection volume and vertebral bone mineral density,which is related to the vertebral biomechanical properties and the curative effect.In clinical practice,the bone cement perfusion volume should be selected according to different bone densities,in order to optimal restoration of the strength and stiffness of the fractured vertebral body,providing reliable spinal stability;preventing postoperative aggravation of vertebral degeneration and secondary fractures in adjacent segments,reducing the complications of intervertebral disc degeneration caused by bone cement leakage,and realizing satisfactory biological mechanical effect and the clinical effect.
