摘要
目的探讨视频脑电图监测对颅脑外伤后继发癫痫发作的早期诊断价值。方法选择2020年7月—2022年8月六盘水市首钢水钢医院收治的84例颅脑外伤后继发癫痫发作患者,随机分为常规组和观察组各42例。常规组采用常规脑电图监测,观察组采用视频脑电图监测,比较两组异常脑电信号、痫样放电、同步发作的检出率及脑叶皮质区、深部白质区的有癫痫样波率。结果观察组异常脑电信号检出率、痫样放电检出率、同步发作检出率、脑叶皮质区的有癫痫样波率、深部白质区的有癫痫样波率分别为61.90%、38.10%、28.57%、35.71%和28.57%,高于常规组的30.95%、7.14%、4.73%、4.73%、4.73%,差异有统计学意义(P<0.05)。结论应用视频脑电图对颅脑外伤后继发癫痫发作患者进行监测,能够提高异常脑电信号的检出率,并且能够明确临床分型。
Objective To analyze the value of video electroencephalogram monitoring in early diagnosis of secondary epileptic seizure after craniocerebral trauma.Methods A total of 84 patients with secondary seizures after craniocerebral trauma admitted to Shuigang Hospital in Liupanshui City from July 2020 to August 2022 were randomly divided into conventional group and observation group,with 42 cases in each group.The conventional group was monitored by routine electroencephalogram,and the observation group was monitored by video electroencephalogram.The detection rate of abnormal electroencephalogram,epileptic discharge,synchronous seizure and epileptic wave in cortex and deep white matter were compared between the two groups.Results The detection rates of abnormal electroencephalogram,epileptic discharge,synchronous seizure,epileptiform wave in cortex,and epileptiform wave in deep white matter were 61.90%,38.10%,28.57%,35.71%and 28.57%in observation group,which were significantly higher than 30.95%,7.14%,4.73%,4.73%and 4.73%in conventional group(P<0.05).Conclusion The application of video electroencephalogram to monitor the patients with secondary epileptic seizure after craniocerebral trauma can improve the detection rate of abnormal electroencephalogram and define the clinical classification.
