摘要
目的:基于数据挖掘的方法研究分析陈贵珍教授治疗围绝经期综合征的用药规律。方法:收集陈贵珍教授于2016年1月—2023年10月治疗PMS的有效临床处方并建库。利用Microsoft Excel 2016进行频次分析,包括四气、五味、归经的使用频次;采用SPSS Modeler 18.0中的网络图形以及Apriori算法根据设置条件进行中药关联规则分析;应用统计软件SPSS26.0对排名前20位高频中药进行聚类分析。结果:共收集有效处方239首,涉及236位患者,使用中药191味。频数分析结果显示,出现频次最高的症状为失眠、月经紊乱、情绪障碍;用药频次前5位依次为茯苓、山药、山萸肉、炒酸枣仁、炙甘草,归经以肝、脾、肾经为主,药性以温为主,药味以甘为主,关联规则分析结果获得药物组合48个,以山药-山萸肉-茯苓为代表;聚类分析显示大致可分为3类,以六味地黄丸、柴胡疏肝散、逍遥散组方的药物为主。结论:陈贵珍教授认为围绝经期综合征的基本病机为肾虚癸竭、脾虚肝郁,治以补肾益癸、健脾和癸、疏肝条癸为法,常用六味地黄汤、柴胡疏肝汤、逍遥散汤剂加减。
Objective:To study the medication rules of Professor CHEN Guizhen treating perimenopausal syndrome based on data mining.Methods:The valid clinical prescriptions of Professor CHEN in the treatment of perimenopausal syndrome from January 2016 to October 2023 were collected and the database was established.Microsoft Excel 2016 was used for use frequency analysis of four qi,five flavors and meridian tropism;the network graphics in SPSS Modeler 18.0 and Apriori algorithm were used to analyze the association rules of Chinese medicine according to the setting conditions;the statistical software SPSS26.0 was used for cluster analysis of the top 20 high-frequency Chinese medicinals.Results:A total of 239 valid prescriptions were collected,involving 236 patients,and 191 Chinese medicinals were used.The results of frequency analysis showed that the symptoms with the highest frequency were insomnia,menstrual disorders and emotional disorders;the top five medicinals in frequency were Poria,Dioscoreae Rhizoma,Corni Fructus,Fried Ziziphi Spinosae Semen and Glycyrrhizae Radix et Rhizoma Praeparate Cum Melle,the main channel entries are the liver,spleen and kidney,and the medicinals were mainly warm in nature and sweet in flavor.The results of association rule analysis suggested 48 medicinal combinations,represented by Dioscoreae Rhizoma-Corni Fructus-Poria;the cluster analysis showed that the medicinals used could be roughly divided into three categories,mainly including the medicinals composed of Liuwei Dihuang Pills,Chaihu Shugan San and Xiaoyao San.Conclusion:Professor CHEN believes that the basic pathogenesis of perimenopausal syndrome is kidney deficiency and kidney essence exhaustion,and spleen deficiency and liver depression.The treatment method should be tonifying kidney and replenishing kidney essence,fortifying spleen and harmonizing kidney essence,and soothing liver and regulating kidney essence,and the modified Liuwei Dihuang Pills,Chaihu Shugan San and Xiaoyao San are commonly used.
