摘要
疫病在给人类造成严重负面影响的同时,也促进了医学的发展。纵观中医发展史,从《黄帝内经》的“百病生于气”“三年化疫”,到《伤寒杂病论》对寒疫的详实记载,再到《温疫论》的“杂气”致疫,各时代对疫病致病因素的认识在逐步加深。几千年来,中医在疫病的防治实践中积累了丰富的经验,形成成熟理论,留下的大量著作,是世界其他任何民族无法相比的。基于此,文章探讨了古代医家对疫病致病因素的理解和认识,首先,阐述疫病的概念和特征,并引用了多个历史文献中的记载来证明疫病具有强烈的传染性和流行性。其次,阐述古代医家关于疫病致病因素的六种观点:六气致疫、伏气致疫、杂气致疫、毒气致疫、虫邪致疫、温邪致疫。在六气致疫中,古代医家详细讨论了寒、火、燥、湿四种致病因素的作用机制。对于伏气致疫的观点,列举了几种不同的病邪伏藏在人体内的位置和发作条件,如伏寒藏于骨髓、肾,伏寒藏于肌肤等,以及热伏于经、血秽伏于命门、热毒伏于气血、胎毒伏于脾肺、疫气伏于膜原以及温毒遇温伏发等多种致病因素。并且,古代医家在阐述各种致疫因素的具体表现的同时,还提供了相应的治疗方法。明确病因是中医辨证论治的基础,横向挖掘疫病致病因素能更好地传承中医疫病理论精华,以期启发现代传染病的防控。文章为了解古代医家对疫病致病因素的理解和分类提供了一个全新的视角,对于理解和预防疫病具有重要的参考价值。
Epidemic diseases has brought severe negative impacts on human beings,while also promotes the development of medicine.Throughout the history of traditional Chinese medicine,from“All diseases are caused by Qi(气)”and“the three-year epidemic transformation”in Huangdi Neijing(《黄帝内经》),to the detailed record of cold epidemic in Shanghan Zabing Lun(《伤寒杂病论》),and then to the“miscellaneous Qi causes epidemic diseases”in Wenyi Lun(《温疫论》),the understanding of pathogenic factors of epidemic diseases has gradually deepened in each era.For thousands of years,traditional Chinese medicine has accumulated rich experience in the prevention and control of epidemics,forming mature theories and leaving behind a large number of works,which is unparalleled by any other nation in the world.Based on this,this paper discusses the understanding and cognition of ancient doctors on the pathogenic factors of epidemic diseases.Firstly,it expounds the concept and characteristics of epidemic diseases,and cites several historical records to prove the strong contagiousness and prevalence of epidemic diseases.Secondly,the author expounds ancient doctors’six viewpoints regarding the causes of epidemic diseases:the six pathogenic factors causing epidemics,the latent pathogenic factors causing epidemics,the miscellaneous pathogenic factors causing epidemics,the pests causing epidemics,and the warm pathogens causing epidemics.The mechanism of the four pathogenic factors of cold,fire,dryness and dampness was discussed in details.For the viewpoint of epidemic disease caused by latent pathogenic factors,it lists the location and attack conditions of several different pathogenic pathogens hiding in the human body,such as latent cold hiding in the bone marrow and kidney,latent cold hiding in the skin,etc.,as well as a variety of pathogenic factors,such as heat hiding in the meridian,blood drooping in the life gate,heat poison hiding in Qi and blood,fetal poison hiding in the spleen and lung,pestilence Qi hi
