摘要
目的采用两样本孟德尔随机化(MR)方法探究抑郁情绪与胃溃疡(GU)之间的因果关联。方法以抑郁情绪与GU形成的全基因组关联研究数据库为基础,筛选出单核苷酸多态性位点(SNPs),设置参数为P<5×10-8,物理遗传距离>10000 kb,连锁不平衡r2<0.001。主要采用随机效应模型的逆方差加权法(MR-IVW)进行分析,并以加权中位数法(WME)、MR-Egger回归法、基于众数的简单估计和基于众数的加权估计进行补充,以OR值及95%CI评价抑郁情绪与GU之间的因果关系,利用异质性检验、水平多效性、逐个剔除检验等进行敏感性分析。结果筛选到60个相关SNPs。IVW结果(OR=1.0043,95%CI=1.0012~1.0073,P=0.007)、WME结果(OR=1.0046,95%CI=1.0004~1.0089,P=0.032)、基于众数的简单估计结果(OR=1.011,95%CI=1.0007~1.0215,P=0.04)、基于众数的加权估计结果(OR=1.0112,95%CI=1.0017~1.0208,P=0.024)均表明,基因预测抑郁情绪与GU的发病风险因素显著相关。MR-Egger回归结果(P=0.163)表明研究结果不存在基因水平多效性。敏感性分析显示研究结果没有产生偏倚,异质性检验表明SNPs不存在异质性。结论抑郁情绪与GU发病存在因果关联,积极进行郁情绪的疏导或可有利于GU的早期发现与及时干预。
Objective A two-sample Mendelian randomization(MR)method was used to investigate the causal relationship between depression and gastric ulcer(GU).Methods Single nucleotide polymorphisms(SNPs)were selected based on the genome-wide association database of depression and GU formation,and the parameters were set as P<5×10-8,physical genetic distance>10000 kb,linkage imbalance r2<0.001.The Mendelian randomization inversevariance weighted(MR-IVW)of the random effect model is mainly used for analysis,supplemented by weighted median method(WME),MR-Egger regression method,simple model-based estimation and model-based weighted estimation.The OR value and 95%CI are used to evaluate the causal relationship between depression and GU.Heterogeneity test,horizontal pleiotropy test and one by one elimination test were used for sensitivity analysis.Results A total of 60 related SNPs were screened.IVW results(OR=1.0043,95%CI=1.002-1.0073,P=0.007),WME results(OR=1.0046,95%CI=1.0004-1.0089,P=0.032),simple model-based estimation results(OR=1.0110,95%CI=1.0007-1.0215,P=0.04)and modal weighted estimation results(OR=1.0112,95%CI=1.0017-1.0208,P=0.024)showed that gene prediction of depression were significantly associated with the risk factors of GU.The result of MR-Egger regression(P=0.163)showed that there was no pleiotropy at the gene level.Sensitivity analysis showed no bias,and heterogeneity test showed no heterogeneity in SNPs.Conclusion There is causal relationship between depression and the onset of GU.Actively channelling depression may be beneficial to the early detection and timely intervention of GU.
