摘要
目的:探讨基于家庭医生巡诊的多元化健康干预对农村老年糖尿病患者大血管病变危险因素的影响。方法:收集2022年8—10月在上海市浦东新区祝桥社区卫生服务中心就诊的农村老年糖尿病患者100例,采用随机数字表法将其分为研究组(n=50)和对照组(n=50)。给予研究组患者基于家庭医生巡诊的多元化健康干预;给予对照组患者常规的社区健康管理。干预前后比较两组患者大血管病变危险因素的相关指标和糖尿病自我管理行为量表得分。结果:干预1年后,对照组患者空腹血糖、血压较本组干预前有所下降,差异有统计学意义(P<0.05)。研究组患者空腹血糖、糖化血红蛋白、血脂、血压、体质指数(BMI)、腰围和糖尿病自我管理行为量表得分均优于本组干预前及对照组干预后,差异有统计学意义(P<0.05)。结论:基于家庭医生巡诊的多元化健康干预能有效改善农村老年糖尿病患者的自我管理行为,从而更好地控制血糖、血脂、血压和肥胖等大血管病变危险因素。
OBJECTIVE To explore the impact of diversified health intervention based on family doctor visits on risk factors of macrovascular disease(MA)in rural elderly patients with diabetes.METHODS 100 rural elderly patients with diabetes who visited Zhuqiao Community Health Service Center in Pudong New District,Shanghai from August 2022 to October were collected.The patients were divided into an experimental group and a control group with 50 cases each using the random number table method.The experimental group applied diversified health intervention based on family doctor visits;the control group applied conventional community health management.Related indicators of MA risk factors of the two groups of patients were compared.RSULTS After 1 year of intervention,the fasting blood glucose and blood pressure of the control group were declined compared with those before the intervention,and the difference was statistically significant(P<0.05).The fasting blood glucose,glycosylated hemoglobin,blood lipids,blood pressure,BMI,waistline and the SDSCA score of the experimental group were all better than those before the intervention in this group and after the intervention in the control group,and the differences were statistically significant(P<0.05).CONCLUSION Diversified health intervention based on family doctor visits can effectively improve the self-management behavior of rural elderly patients with diabetes,thereby better controlling MA risk factors such as blood sugar,blood lipids,blood pressure and obesity.
