慢性乙型肝炎患者血清CCL19、COMP水平与HBV-DNA载量及肝硬化的关系

Correlations of serum levels of CCL19 and COMP with HBV-DNA load and liver cirrhosis in patients with chronic hepatitis B

摘要 目的探讨慢性乙型肝炎患者血清趋化因子配体19(CCL19)、软骨寡聚基质蛋白(COMP)水平与乙型肝炎病毒(HBV)-DNA载量及肝硬化的关系。方法回顾性抽取2021年3月至2023年3月郑州大学第一附属医院收治的96例慢性乙型肝炎患者为研究组,其中非肝硬化者40例,肝硬化者56例。另抽取同期85例体检健康者作为对照组。采用酶联免疫吸附法检测CCL19、COMP水平,采用实时荧光定量法检测HBV-DNA载量。比较研究组与对照组、研究组中非肝硬化者与肝硬化者各指标的表达情况。采用Pearson分析CCL19、COMP水平与HBV-DNA载量的相关性,采用受试者工作特性曲线下面积(AUC)评估血清CCL19、COMP水平对慢性乙型肝炎进展为肝硬化的预测价值。结果研究组血清CCL19、COMP水平高于对照组(P<0.05),研究组中肝硬化者血清CCL19、COMP水平及HBV-DNA载量高于非肝硬化者(P<0.05)。慢性乙型肝炎患者血清CCL19、COMP水平与HBV-DNA载量呈正相关(r=0.50、0.49,P=0.015、0.004)。血清CCL19和COMP水平预测慢性乙型肝炎进展为肝硬化的AUC(95%CI)分别为0.74(0.712~0.836)、0.87(0.813~0.917),两项联合预测的AUC(95%CI)为0.92(0.875~0.955)。结论血清CCL19、COMP高表达与慢性乙型肝炎患者发生肝硬化有关,与HBV-DNA载量呈正相关,其对肝硬化风险具有一定的预测价值。 Objective To investigate the correlations of serum levels of chemokine ligand 19(CCL19)and cartilage oligomeric matrix protein(COMP)with hepatitis B virus(HBV)-DNA load and liver cirrhosis in patients with chronic hepatitis B.Methods A total of 96 patients with chronic hepatitis B admitted to the First Affiliated Hospital of Zhengzhou University from March 2021 to March 2023 were retrospctively selected as the study group,including 40 cases without cirrhosis and 56 cases with cirrhosis.Another 85 healthy people who underwent physical examination during the same period were selected as the control group.The levels of CCL19 and COMP were detected by enzyme-linked immunosorbent assay,and the HBV-DNA load was detected by real-time fluorescence quantification.The CCL19 expression,COMP expression and HBV-DNA load were compared between the study group and the control group,and between the cirrhosis patients and the non-cirrhosis patients in the study group.Pearson analysis was used to analyze the correlation between levels of CCL19 and COMP with HBV-DNA load.The area under the receiver operating characteristic curve(AUC)was used to evaluate the value of serum levels of CCL19 and COMP in predicting the progression of chronic hepatitis B to cirrhosis.Results The serum levels of CCL19 and COMP in the study group were higher than those in the control group(P<0.05);and the serum level of CCL19,serum level of COMP and HBV-DNA load in patients with cirrhosis were higher than those in patients without cirrhosis in the study group(P<0.05).The serum levels of CCL19 and COMP were positively correlated with HBV-DNA load in patients with chronic hepatitis B(r=0.50,0.49;P=0.015,0.004).The AUC(95%CI)of serum levels of CCL19 and COMP in predicting the progression of chronic hepatitis B to cirrhosis were 0.74(0.712-0.836)and 0.87(0.813-0.917),respectively;the AUC(95%CI)of the combination of the two indexes was 0.92(0.875-0.955).Conclusions The high expression of serum CCL19 and COMP are associated with development of cirrhosis in patien
