抗CCP抗体、CCL19、COMP在类风湿关节炎中的表达及临床意义 被引量:8

摘要 目的:探究抗环瓜氨酸肽(CCP)抗体、趋化因子配体19(CCL19)、软骨寡聚基质蛋白(COMP)在类风湿关节炎(RA)中的表达及临床意义。方法:选取84例RA患者为RA组,78例同期其他风湿病患者为其他风湿病组,72名同期体检健康志愿者为健康对照组。比较3组对象血清抗CCP抗体、CCL19、COMP水平,受试者工作特征(ROC)曲线分析其对RA的诊断价值;比较不同血清抗CCP抗体、CCL19、COMP水平RA患者晨僵时间、疾病活动度[28个关节疾病活动度评分(DAS28)]、血沉(ESR)、C反应蛋白(CRP)、类风湿因子(RF)等临床及实验室指标,分析血清抗CCP抗体、CCL19、COMP水平与RA患者各项临床及实验室指标的相关性。结果:3组对象血清抗CCP抗体、CCL19、COMP水平比较,差异有统计学意义(P<0.05),且RA组>其他风湿病组>健康对照组。ROC曲线分析显示,血清抗CCP抗体、CCL19、COMP对RA均具有较高诊断价值(P<0.05),截断值分别为57.930 RU/mL、235.280 pg/mL、25.685 ng/mL。高血清抗CCP抗体者、高血清CCL19者及高血清COMP者晨僵时间、DAS28评分及ESR、CRP、RF水平高于低血清抗CCP抗体者、低血清CCL19者、低血清COMP者(P<0.05)。相关性分析显示,血清抗CCP抗体、CCL19、COMP水平与RA患者晨僵时间、DAS28评分及ESR、CRP、RF水平呈正相关(P<0.05)。结论:血清抗CCP抗体、CCL19、COMP不仅能辅助诊断RA,还能评估RA病情,对改善RA诊治现状有积极意义。 Objective:To explore the expressions and clinical significance of anti-cyclic citrullinated peptide(CCP)antibody,chemokine ligand 19(CCL19)and cartilage oligomeric matrix protein(COMP)in rheumatoid arthritis(RA).Methods:The clinical data of 84 RA patients(RA group)were retrospectively analyzed.The data of 78 patients with other rheumatic diseases(other rheumatosis group)and 72 healthy examiners(healthy control group)were collected.The differences of serum anti-CCP antibody,CCL19 and COMP were compared among the three groups.The receiver operating characteristic curve(ROC curve)was used to evaluate the diagnostic value of serum anti-CCP antibody,CCL19 and COMP on RA.The clinical and laboratory indicators such as morning stiffness time,disease activity[28 joint disease activity score(DAS28)],erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and rheumatoid factor(RF)were compared among RA patients with different levels of serum anti-CCP antibody,CCL19 and COMP.The correlation between levels of serum anti-CCP antibody,CCL19 and COMP and clinical and laboratory indicators of RA patients was analyzed.Results:The levels of serum anti-CCP,CCL19 and COMP in the three groups showed RA group>other rheumatosis group>healthy control group(P<0.05).ROC curve analysis showed that the serum anti-CCP antibody,CCL19 and COMP were found to have high diagnostic value on RA(P<0.05),and the cut-off values were 57.930 RU/ml,235.280 pg/ml and 25.685 ng/ml respectively.The morning stiffness time,DAS28 score and levels of ESR,CRP and RF in patients with high serum anti-CCP antibody were higher than those of patients with low serum anti-CCP antibody(P<0.05),and the above indexes in patients with high serum CCL19 were higher than those in patients with low serum CCL19(P<0.05),and the above indexes in patients with high serum COMP were higher than those in patients with low serum COMP(P<0.05).Pearson correlation analysis showed that serum anti-CCP antibody,CCL19 and COMP were positively correlated with morning stiffness time,DAS28 sc
