摘要
目的 探讨分析十味龙胆花颗粒配合抗菌药物治疗痰热壅肺证型社区获得性肺炎(CAP)患者的疗效研究。方法 选取2022年2月至2023年2月吉安市中心人民医院收治的痰热壅肺证型CAP患者80例,根据随机数字表法将患者分为对照组(接受抗菌药物治疗),联合组(在对照组的基础上给予十味龙胆花颗粒治疗),每组患者各40例,两组均持续治疗7 d。比较两组临床疗效、症状改善时间、社区获得性肺炎评分(CURB-65评分)、免疫功能、血清炎性因子水平变化、不良反应。结果 联合组的临床疗效总有效率92.50%高于对照组总有效率75.00%(P<0.05)。治疗后,联合组的发热、咳痰、气促、肺部啰音等症状改善时间短于对照组(P<0.05);联合组的CURB-65评分低于对照组(P<0.05);联合组的免疫功能CD3^(+)、CD4^(+)水平高于对照组,CD8^(+)水平低于对照组,差异有统计学意义(P<0.05);联合组的血清IL-8、Ang-2、C反应蛋白水平低于对照组(P<0.05)。对照组不良反应发生率为10.00%,联合组不良反应发生率为5.00%,两组相比差异无统计学意义(P>0.05)。结论 十味龙胆花颗粒联合抗菌药物治疗痰热壅肺证型CAP,能够有效地改善患者的临床症状和免疫功能,临床疗效较好,同时具有较高的安全性。
Objective To investigate the efficacy of shiwei longdanhua granules combined with antibiotics in the treatment of phlegm heat obstructing lung syndrome type community acquired pneumonia(CAP)patients.Methods A total of 80 patients with CAP admitted to Ji'an Central People's Hospital from February 2022 to February 2023 were selected and divided into control group(receiving antibiotics treatment)and combined group(receiving shiwei longdanhua granules treatment on the basis of control group)according to random number table method,with 40 patients in each group.Both groups were treated continuously for 7 days.The clinical efficacy,symptom improvement time,community acquired pneumonia score(CURB-65 score),immune function,changes in serum inflammatory factors and adverse reactions were compared between the two groups.Results The total effective rate of combined group 92.50%was higher than that of control group 75.00%,the difference was statistically significant(P<0.05).After treatment,the improvement time of fever,expectoration,shortness of breath and pulmonary rales in the combined group were significantly shorter than that in the control group,the difference were statistically significant(P<0.05);the CURB-65 score of the combined group was significantly lower than that of the control group,with statistical significance(P<0.05);the levels of immune function CD3^(+)and CD4^(+)in combined group were significantly higher than those in control group,and CD8^(+)levels was significantly lower than that in control group,the difference was statistically significant(P<0.05);the levels of IL-8,Ang-2 and C-reactive protein in combined group were significantly lower than those in control group,with statistical significance(P<0.05).The incidence of adverse reactions was 10.00%in control group and 5.00%in combined group,and there was no statistical significance between the two groups(P>0.05).Conclusion Shiwei longdanhua granules combined with antibiotics can effectively improve the clinical symptoms and immune function for the treat
