摘要
目的 探讨左卡尼汀联合重组人促红素注射液对血液透析患者钙磷代谢及全段甲状旁腺激素(iPTH)的影响。方法 按照随机数字表法将2019年6月至2023年4月于我院行血液透析治疗的66例患者分为2组。对照组33例予以人促红素注射液,观察组33例基于此加用左卡尼汀。比较2组治疗效果、钙磷代谢、铁蛋白(SF)以及iPTH水平。结果 观察组治疗效果优于对照组,差异有统计学意义[97%(32/33)与76%(25/33),χ^(2)=4.632,P=0.031]。观察组治疗后血磷水平(1.57±0.31)mmol/L低于对照组(2.36±0.42)mmol/L;血钙水平(2.29±0.47)mmol/L高于对照组(1.91±0.43)mmol/L,差异有统计学意义(P<0.05)。2组治疗前SF、iPTH未见差异,但观察组治疗后SF均高于对照组,iPTH低于对照组,差异有统计学意义(P<0.05)。结论 血液透析患者采用左卡尼汀联合重组人促红素注射液治疗效果确切,能够增强治疗效果,改善钙磷代谢,降低iPTH水平,调节铁代谢指标。
Objective To investigate the effect of levocarnitine and human erytropin injection on calcium and phosphorus metabolism and complete parathyroid hormone(iPTH)in hemodialysis patients.Methods Sixty-six patients who underwent hemodialysis from June 2019 to April 2023 were divided into 2 groups according to the random number table method.Control group of 33 patients received human erythropin injection,based on this,33 patients in the observation group added levocarnitine.The treatment effects,calcium and phosphorus metabolism,ferritin(SF),and iPTH levels were compared in the two groups.Results The treatment effect of the observation group was better than that of the control group,with a statistical difference[97%(32/33)vs 76%(25/33),χ^(2)=4.632,P=0.031].In the observation group(1.57±0.31)mmol/L was lower than the control group(2.36±0.42)mmol/L,and the blood calcium level(2.29±0.47)mmol/L was higher than the control group(1.91±0.43)mmol/L,showing a significant difference(P<0.05).There was no difference in SF and iPTH before the two groups,but the SF was higher than the control group and iPTH was lower than the control group,which was statistically significant(P<0.05).Conclusion In hemodialysis patients,levocarnitine and human erytropin injection are effective,which can enhance the treatment effect,improve calcium and phosphorus metabolism,reduce iPTH level,and regulate iron metabolism index.
